Maximal tachycardia and high cardiac strain during night shifts of emergency physicians

Abstract: We demonstrated several incidences of maximal HR during shifts combined with a high cardiac strain. Duration of tachycardia were the highest in 24hS and lasted several hours. Such values are comparable to those of workers exposed to high physical demanding tasks or heat. Therefore, we suggest that EPs limit their exposure to 24hS. We, furthermore, demonstrated benefits of HR monitoring for identifying stressful events. ClinicalTrials.gov identifier: NCT01874704.

“…The Socio-demographic characteristics questionnaire assessed age, gender, weight, size, occupational group, education level, family situation, work characteristics, physical activity, and quantity of sleep. Sleep quality, well-being and perceived stress at work and at home are evaluated using visual analogue scales (VAS), i.e., by moving a cursor on a horizontal, non-calibrated line of 10 cm, ranging from very low (0) on the left to very high (10) on the right [ 44 , 45 , 46 , 47 , 48 , 49 ]. We also looked for the presence of other addictions such as tobacco, cannabis, and alcohol.…”

“…Out of these eight selected clinical trials, four of them assessed the role of shift work as a cause of circadian misalignment in cardiac diseases [6,7,8,9]. Five among them assessed the sleeprelated issues like insomnia, obstructive sleep apnea and lack of sleep as a contributing factor in cardiac diseases [8,10,11,12,13].…”

“…During normal night sleep, the sympathetic nervous system is at its nadir. During a busy night shift in the ED, the converse is true: Studies of emergency physicians working at night have shown elevated heart rate, blood pressure, stress response, and prevalence of tachydysrhythmias [53-55]. The association of night shift work and increased risk of certain cancers has led to the International Agency for Research on Cancer to classify shift work that involves circadian disruption as a probable human carcinogen [56,57].…”
