A mandíbula é um dos ossos da face mais acometidos nos casos de trauma maxilofacial e os projéteis por arma de fogo encontram-se dentre os agentes etiológicos mais comuns. Seu tratamento é desafiador no âmbito da Cirurgia Bucomaxilofacial tendo em vista a alta vascularização da cabeça e do pescoço e o grau de cominuição óssea que essas fraturas podem apresentar. Sendo assim, o objetivo deste trabalho é relatar um caso clínico sobre o manejo cirúrgico de fratura mandibular cominutiva, bilateral, provocada por projétil de arma de fogo (PAF). Paciente do sexo feminino, 33 anos de idade, vítima de ferimento por PAF, compareceu ao Hospital Geral do Estado da Bahia cursando com fraturas cominutivas em região de ângulo mandibular bilateral; ramo e corpo mandibular direito. A paciente foi submetida à cirurgia sob anestesia geral como forma de tratamento e os cotos ósseos fraturados foram fixados com miniplacas do sistema 2.0 mm e placas de reconstrução do sistema 2.4 mm. Além disso, elásticos foram instalados para guia de oclusão. O tratamento das fraturas mandibulares por PAF podem variar de conservador à grandes reconstruções. A escolha da conduta ideal é bastante controversa, mas normalmente é baseada na experiência do cirurgião e, principalmente, na complexidade da fratura, a qual será diagnosticada a partir de um exame clínico bem detalhado, associado a exames de imagem. O tratamento cirúrgico precoce desse tipo de trauma permite um baixo índice de complicações e sobretudo o retorno antecipado do paciente às atividades diárias.

Firearm injuries are now the leading cause of pediatric mortality in the United States. With the number of firearm injuries increasing at an alarming rate, the American Medical Association (AMA) declared firearm violence a public health crisis. In response to this emerging public health issue, the American College of Surgeons (ACS) developed the STOP THE BLEED training to educate laypersons on how to mitigate acute hemorrhage following gunshot wounds (GSWs) and other ballistic injuries. Stabilization of patients following GSWs is often handled by a multidisciplinary team of trauma and reconstructive surgeons. Here, we describe the history and ongoing role of reconstructive surgeons in preventing and addressing firearm morbidity and mortality. Hand surgeons are uniquely positioned to counsel patients on firearm safety, e.g., educating patients on proper firearm storage away from minors in the home, in an effort to mitigate accidental firearm injury to the upper extremity. As the evolving climate of firearm violence continues to rise, plastic and reconstructive surgeons will continue to play a critical role in restoring form and function among patients afflicted with GSWs.

Treatment of gunshot wounds in the maxillofacial region is complex. Current literature supports immediate treatment. Wounds vary widely. The nature of the injuries must be carefully assessed using the Kanzanjian and Converse’s principles of plastic surgery as guidance, but always adapting to specific needs. Management is dependent upon the type of weapon, the bullet’s characteristics, kinetic energy, place of impact, as well as the patient’s general health status. A case is presented of a gunshot injury to the face with a review of the literature.
