Background Midpalatal suture (MPS) maturation may be influenced by a range of parameters including age, gender, and vertical skeletal pattern. We therefore aimed to evaluate the effect of Frankfurt-mandibular angle (FMA), skeletal maturation, and age on the timing of MPS maturation. Methods In this cross-sectional study, cone-beam computed tomography (CBCT) and lateral cephalograms were used to assess the MPS and cervical vertebral maturation (CVM) stage. A proportional odds logistic regression model was used to assess associations between age adjusted for gender and MPS maturation, and a regression analysis was performed to analyze the effect of vertical pattern on these associations. Results A total of 201 patients (84 male and 117 female) with a mean age of 13.48 (SD 1.94) were included. With increasing age, the odds of belonging to a higher maturation stage increased (OR: 2.14; 95% CI 1.789; 2.567; P < 0.001); however, no association between FMA and MPS maturation was observed (OR: 1.01; 95% CI 0.964; 1.051; P = 0.76). A strong correlation between MPS maturation and CVM stage was not reported. Males had a higher probability of belonging to a lower MPS maturation stage (OR: 0.24; 95% CI 0.136; 0.415; P < 0.001). Conclusions Based on this cross-sectional analysis, midpalatal sutural maturation classification is associated with chronological age and occurs later in males. Neither CVM staging nor variation in vertical skeletal proportions were useful predictors of midpalatal maturation stage.

Introducción: La evaluación de la edad esquelética es un factor importante en la planificación ortodóncica para anticipar cambios en el crecimiento y es el análisis de radiografías de mano y muñeca el que muestra el grado del potencial de crecimiento óseo y facial. El objetivo fue evaluar la relación entre la maduración esquelética de la mano y muñeca, y la osificación de sutura media palatina (SMP) en adolescentes. Materiales y métodos: Se realizó una búsqueda en cuatro bases de datos: PubMed, Scopus, ScienceDirect y Embase, revisadas hasta el 13 de diciembre de 2022. Los estudios incluidos fueron artículos descriptivos y comparativos de la maduración esquelética de la mano y muñeca, y osificación de la sutura media palatina de pacientes de 7 a 18 años. Dos investigadores seleccionaron cuidadosamente los artículos evaluados y analizaron los diferentes tópicos clave relacionados con el tema. Resultados: En este estudio se incluyeron 4 artículos. Según los estudios, se encontró que mientras mayor sea el grado de maduración ósea hay un aumento en la aproximación de la SMP, especialmente en etapas tardías, con correlaciones altas y positivas; además, hubo mayores resultados de correlación con el método de análisis de Fishman, a diferencia de los métodos de Hagg y Taranger y Björk. En los estadios de límite critico en SMI7-9, se encontró mayor acercamiento al cierre de SMP compatibles con estadio D-E. La finalización de la maduración en mujeres se completó hasta en 2 años antes que en los hombres. Conclusiones: Los métodos de evaluación para el diagnóstico mediante el análisis carpal puede usarse para las evaluaciones de predicción del estadio de maduración de la SMP; sin embargo, los resultados no fueron absolutos en todos los casos, por lo que no se puede generalizar.

Introduction. One of the main auxiliary methods for making a final diagnosis and choosing treatment plan in the clinical practice of an orthodontist is X-ray, which can be used at all stages of dental rehabilitation. The purpose of this study is to elaborate and justify the criteria for a personalized approach in planning X-ray examination of orthodontic patients based on literary analysis and own clinical experience. Materials and methods. We conducted a thorough examination of relevant literature using key words such as X-ray methods, dentistry, orthodontics, orthopantomogram, cephalometric analysis, cone-beam computed tomography, and ionizing radiation. We performed a retrospective analysis of 1054 medical records of patients received treatment for the period from 2020 to 2023. Special attention was given to the prescribed type of X-ray examinations and findings obtained. Results. Our analysis encompassed an examination of legal documents governing the radiation safety of Ukrainian citizens, including the optimal annual dose resulting from medical examinations or treatments. We conducted a comparative assessment of ionizing radiation levels in patients undergoing various types of X-ray examinations using commonly employed X-ray equipment. We established indications and visualization zones for each radiological examination method utilized in orthodontic cases, comprising 2-D techniques (orthopantomogram, teleroentgenogram of the head in direct and lateral projections) and 3-D methods (cone-beam computed tomography of the segment, jaw, and the facial region of the skull). Conclusion. The personalized approach criteria encompass selecting an area of special interest based on clinical assessments, the capacity to analyze and determine the sequence of treatment measures through an analysis of the acquired data, and a deep understanding of radiation safety principles for the well-being of patients.
