Maternal Serum Soluble Endoglin at 30-33 Weeks in the Prediction of Preeclampsia

Abstract: acteristics and third-trimester sEng, the detection rates of intermediate-and late-PE, at a false-positive rate of 10%, were 64.3 and 50.0%, respectively. Conclusion: Screening by maternal characteristics and sEng at 30-33 weeks could identify most pregnancies that will subsequently develop PE.

“…This research has already started. This issue of Fetal Diagnosis and Therapy publishes four studies performed by the Fetal Medicine Foundation which provide important evidence supporting that prediction of late-onset PE is possible by means of either uterine artery Doppler or angiogenic factors [9,10,11,12]. These results are in line with another very recent study from the United States [13].…”

“…First- and second-trimester strategies show poorer predictive values for late-onset forms of PE and IUGR [51,52,53,54,55,56,57]. This may be explained by the milder nature of the disease, where changes can possibly not be picked up that early in gestation, together with the heterogeneity of these clinical conditions late in pregnancy.…”

“…Predictive values in the third trimester for late PE are superior to those achieved in the first or second trimester of pregnancy. In addition, several studies from the Fetal Medicine Foundation [55,56] have also evaluated the predictive value of angiogenic factors together with maternal characteristics and uterine Doppler in the third trimester for late PE. A combination of maternal characteristics and serum PlGF had a detection rate of 86 and 53% for intermediate PE (delivery at 34-37 weeks) and late PE (delivery after 37 weeks), with a 10% false-positive rate.…”

“…61 The predictive performance of biomarkers for late pre-eclampsia is improved if the evaluation is performed in the third trimester. 85,86 However, in most studies, the majority of patients with an imbalance between angiogenic and antiangiogenic factors do not subsequently develop pre-eclampsia. 8187 Such tests, therefore, have low PPVs regardless of the timing of sample collection.…”
