We read with interest the article by Aroviita and colleagues 1 reporting that the concentration of hematopoietic progenitor cells in cord blood (CB) was higher in male infants, even after adjustment for birth weight. Previously, by use of multivariate analysis, we 2 and others 3,4 did not observe an influence of sex on hematopoietic content of CB. In response to the observations of Aroviita and coworkers and recognizing the importance of infants' weight on the volume and cell content of CB, 5 we reanalyzed pertinent data for our stored CB units.We reviewed data obtained before freezing for CB collected from 1016 male (49%) and 1046 female (51%) newborns. The median gestational age of male infants did not differ from that of females (40 ± 1.2 weeks vs. 39.7 ± 1.3 weeks). The median birth weight of male infants (3443.7 ± 483.7 g) was significantly greater than that of female infants (3291.9 ± 435.7 g; p < 0.001). CB collections from male infants had fewer total nucleated cells (TNCs; 132.3 × 10 7 ± 46 × 10 7 vs. 125.5 × 10 7 ± 117 × 10 7 ; p < 0.001), but more lymphocytes (36 ± 8% vs. 34.6 ± 7%; p < 0.005) and a higher percentage of CD34+ cells (0.39 ± 0.27% vs. 0.33 ± 0.2%, p < 0.001). There were more colony-forming units (CFUs) in CB collections from male newborns compared to female newborns (137.8 × 10 4 ± 129 × 10 4 vs. 125.5 × 10 4 ± 117 × 10 4 ; p < 0.05). We also analyzed the content of CB collections by newborns' weight and sex. CB collected from female newborns had more TNCs, fewer lymphocytes, and lower percentages of CD34+ cells than from male newborns of comparable birth weight (Table 1). Although the percentage of CD34+ cells was greater for CB collected from male newborns, except for the highest weight category (>4000 g), there was no sex-related difference in the total CD34+ content for the CB (Table 1). There is increasing scientific evidence that human umbilical cord cells are a valuable source of adult stem cells that can be used for various implications including regenerative medicine and tissue engineering. The review describes the role of progenitor cells (mesenchymal, endothelial, prenatal) for the use in cardiovascular tissue engineering, i.e., the formation of large vessels and heart valves from umbilical cord cells. Currently used replacements in cardiovascular surgery are made of foreign materials with well known drawbacks such as thrombo-embolic complications, infection, loss of functional and biological properties, and others. Especially in the field of replacements in congenital cardiac defects, there would be a need of materials which have the advantage of optimal biological and mechanical properties. In the case of human umbilical cord cells, autologous cells can be used by minimally invasive procedures. The cells have excellent growth capacities and form a neo-matrix with excellent mechanical properties. For optimal growth and modeling, scaffolds are required with high biocompatibility and biodegradability, which allow cell attachment, ingrowth, and organization. Nutrients and waste must be easily transported and cells should be in entire contact with host's body. Finally, regenerated materials can be fully incorporated and the scaffold is completely replaced. Besides these cell and scaffold requirements, feto-maternal conditions and risk factors concerning deriving stem cells are of major interest. There are still many open questions concerning whether and how maternal conditions such as infection (viral or bacterial) or gestational age of the newborn influence stem cell harvesting and quality. If these cells will be used for the construction of replacement materials, it is clear that very strict criteria and protocols be introduced enabling the promising step from isolated cells to a therapeutic device such as a new heart valve. It is hoped that it will be only a question of time until human umbilical cord cells will be used frequently as the source of cardiovascular tissues among others in the clinical setting of treating congenital heart defects.

Llama la atención el hecho que se hubiera encontrado correlación solamente entre la longitud del cordón umbilical y el recuento total de células nucleadas y no también con el recuento de células CD34+, teniendo en cuenta, y según los resultados del presente estudio, la existencia de correlación entre estos dos marcadores de calidad, inquietudes que valen la pena tener en cuenta para próximas investigaciones.…”

ResumenIntroducción: La sangre de cordón umbilical (SCU) ha sido reconocida como una fuente de células madre hematopoyéticas. Múltiples estudios han sido realizados con el propósito de determinar variables maternas y neonatales que afecten el volumen, el recuento total de células nucleadas y de células CD34+. El presente estudio correlaciona variables maternas y neonatales con el recuento total de células nucleadas y de CD34+ medidas en µL (microlitro). Materiales y Métodos:Estudio correlacional en el que se analizaron 50 muestras de sangre de cordón umbilical de gestantes atendidas en dos IPS, una de Bogotá y otra de Ubaté, (Cundinamarca), durante un período de 7 meses del año 2013. La selección de estas muestras fue basada en un muestreo no probabilístico. Se calculó el coeficiente de correlación de Pearson con su respectiva significancia estadística entre las variables cuantitativas de la madre y del neonato, y el volumen, el recuento de células CD34+ y el recuento total de células nucleadas por µL. Resultados:Se encontró correlación positiva entre la longitud del cordón y el recuento total de células nucleadas. Así mismo entre el volumen inicial de la muestra y el recuento total de células nucleadas y el recuento de células CD34+ en µL y en mL (mililitro). Discusión:Múltiples estudios se han desarrollado entorno a la búsqueda de variables maternas y neonatales que afectan la calidad de la muestra. En Colombia no se habían descrito las correlaciones encontradas en el presente estudio. Es por ello, que el desarrollo de nuevos estudios con diseños analíticos será enriquecedor. Multiple studies have been conducted in order to determine maternal and neonatal variables that affect the volume, total nucleated cell count and CD34+ cells. This study correlated maternal and neonatal variables with the total nucleated cell count and CD34+ cells measured by µL. Materials and methods: Correlational study in which 50 samples of umbilical cord blood were analyzed of pregnant women at two health institutions, one at Bogotá and the other one at Ubaté, Cundinamarca, for a period of seven months of 2013. The selection of these samples was based on a non-probability sample. The Pearson correlation coefficient was calculated with their respective statistical significance between quantitative variables of mother and newborn, and the volume, the count of CD34+ cells and the total nucleated cell count measured by µL.
