Zusammenfassung. Grundlagen: Skrotalhernien mit Harnblasenanteilen sind selten, noch seltener ihre Einklemmung. Rezidivhernien nach Lichtenstein-Operation treten häufigerer medial als lateral auf.Methodik (Kasuistik): Wir berichten über einen 73-jährigen Mann, der mit einer indirekten inkarzerierten und rezidivierenden Skrotalhernie vier Jahre nach Lichtenstein-Herniotomie operiert wurde. Bruchinhalt war ein Großteil der Harnblase mit Blasentamponade.Ergebnisse: Die möglichen Gründe des erweiterten inneren Leistenrings werden diskutiert. Zusätzlich zu einem eventuell zu weiten ,,inneren Leistenring'' nach primärer Lichtenstein-Operation könnte ein HarnblasenOutlet-Syndrom z. B. als Folge einer Prostata-Hypertrophie durch erhöhten intraabdominellen Druck kontinuierlich am Netzspalt weiter reißen und so wesentlicher Faktor der Pathogenese einer indirekten Rezidivhernie sein.Schlussfolgerungen: Ein Netzriss durch erhöhten intraabdominellen Druck kann auch Jahre nach Lichtenstein-Operation zu einer indirekten Rezidivhernie beitragen.Schlüsselwörter: Leistenhernie, Rezidiv, Inkarzeration, Harnblase, Harnblasentamponade.Summary. Background: Scrotal herniation of the urinary bladder is rare, there are few case reports about incarceration. Recurrent hernias after Lichtenstein procedure are more often medial than lateral hernias.Methods (case report): We report on a 73-year-old patient presenting a recurrent indirect incarcerated scrotal sliding hernia four years after Lichtenstein procedure. It contained the major part of the urinary bladder with a bladder tamponade.
Results:The possible reasons of the wide internal ring are discussed. In addition to a somewhat wider internal ring of the primary Lichtenstein operation a bladder outlet syndrome e.g. caused by prostate hypertrophy may increase the intra-abdominal pressure resulting in a continuous tearing of the mesh split and thus can be an important factor in the pathogenesis of a recurrent hernia.Conclusions: Mesh rupture may cause recurrent hernias even years after Lichtenstein operation.
