Rationale Patients with type 2 diabetes may co‐ingest herbal and prescription medicines to control their blood sugar levels. Competitive binding of drug and herb may mutually affect their metabolism. This can alter the level of drug and its kinetics in the body, potentially causing toxicities or loss of efficacy. Understanding how the metabolism of sulfonylureas like glyburide and gliclazide can be affected by the presence of berberine and vice versa can provide valuable information on the possible risk of toxicities caused by co‐ingestion of drugs. Methods Berberine and sulfonylureas (glyburide and gliclazide) were co‐incubated with rat liver microsomes in the presence of a NADPH‐regenerating system. The metabolites of berberine and sulfonylureas were analysed using liquid chromatography with high‐resolution mass spectrometry in the positive ion mode. The role of individual isozymes in the metabolism of berberine, glyburide and gliclazide was investigated by using specific inhibitors. Results In vitro metabolism of berberine led to the formation of demethyleneberberine (B1a) and its isomer B1b through demethylenation. Berberrubine (B2a) and its isomer B2b were formed through demethylation. The isozymes CYP3A and CYP2D were found to be involved in the metabolism of berberine. In vitro metabolism of glyburide and gliclazide led to the formation of hydroxylated metabolites. The isozymes CYP3A and CYP2C were found to be involved in the metabolism of glyburide. Gliclazide was metabolised by CYP2C. In vitro co‐incubation of glyburide or gliclazide with berberine showed that each drug's metabolism was compromised as they share a common isozyme. A strong negative linear correlation of glyburide or gliclazide metabolite levels and the concentration of berberine confirmed the effect of berberine on the metabolism of sulfonylureas. Conclusions The metabolism of sulfonylureas and berberine was affected when these compounds were co‐incubated with each other. This may be attributable to competitive binding of the herb and drug to the catalytic sites of the same isozymes.

This work is an attempt to overcome antimicrobial resistance problem which dispersed worldwide inparticular developing countries due to misuse of antibiotics. Actinobacteria were isolated and screenedagainst selected resistant Gram-negative bacteria to detect the powerful antibacterial activity.Identification of the most potent actinobacterial isolate has been carried out using classical and geneticalmethods. Antibacterial compound has been extracted, purified and characterized using accurate and morespecific techniques and instruments. Among forty actinobacterial isolates, only twenty-two isolates couldinhibit the growth of Gram-negative bacteria. The most potent isolate Eg-7 was identified as S.griseorubens, which has a typical 16S rRNA gene. The antibacterial compound was extracted using ethylacetate, and separated by High Performance Liquid Chromatography using methanol and water as amobile phase. Five active peaks were displayed and retained in the range 40 – 45 min, but the last threepeaks were retained at 41.90, 43.43, and 44.54 min, respectively. The crude extract was analyzed byliquid chromatography mass spectrum, where the active peak was displayed at 721.325 m/z. Theantibacterial compound was purified using flash column chromatography and gel filtration columnchromatography. The active fraction was analyzed by Infra-Red spectrum, where a broad absorption at3338 cm-1 was displayed. Molecular formula of an antibacterial compound was determined by massspectrum as C35H26N6O4. Nuclear magnetic resonance analysis was carried out for an antibacterialcompound. These results suggest that a new antibacterial compound that similar quinolone could beproduced by S. griseorubens and exhibited a higher activity against Gram-negative bacteria.

Increased interest over the levels of pharmaceuticals detected in the environment has led to the need for new approaches to manage their emissions. Inappropriate disposal of unused and waste medicines and release from manufacturing plants are believed to be important pathways for pharmaceuticals entering the environment. In situ treatment technologies, which can be used on-site in pharmacies, hospitals, clinics, and at manufacturing plants, might provide a solution. In this study we explored the use of Pyropure, a microscale combined pyrolysis and gasification in situ treatment system for destroying pharmaceutical wastes. This involved selecting 17 pharmaceuticals, including 14 of the most thermally stable compounds currently in use and three of high environmental concern to determine the technology’s success in waste destruction. Treatment simulation studies were done on three different waste types and liquid, solid, and gaseous emissions from the process were analyzed for parent pharmaceutical and known active transformation products. Gaseous emissions were also analyzed for NOx, particulates, dioxins, furans, and metals. Results suggest that Pyropure is an effective treatment process for pharmaceutical wastes: over 99 % of each study pharmaceutical was destroyed by the system without known active transformation products being formed during the treatment process. Emissions of the other gaseous air pollutants were within acceptable levels. Future uptake of the system, or similar in situ treatment approaches, by clinics, pharmacists, and manufacturers could help to reduce the levels of pharmaceuticals in the environment and reduce the economic and environmental costs of current waste management practices.Electronic supplementary materialThe online version of this article (doi:10.1007/s00267-016-0728-9) contains supplementary material, which is available to authorized users.
