Mass-forming pancreatitis: Value of contrast-enhanced ultrasonography

Abstract: CEUS allowed diagnosis of mass-forming pancreatitis with diagnostic accuracy of 96%. CEUS significantly increases the diagnostic confidence with respect to basal US in discerning mass-forming pancreatitis from pancreatic neoplasm.

“…60 Edematous acute pancreatitis appears at a conventional B-mode examination, as widely described before, with a dimensional increase of the gland and a diffusely hypoechoic texture. The injection of contrast agent will show a diffused and homogeneous wash-in of the gland, with different degrees of enhancement, resulting in an increased echogenicity during the dynamic phases.…”

“…CEUS had 88.6% Se, 97.8% Sp, 91.2% PPV, and 96% accuracy for the diagnosis of pseudotumoral CP, while in 94% of cases the inflammatory mass showed moderate enhancement following contrast, similar to the adjacent pancreatic parenchyma [20].…”

“…The US aspect of pseudotumoral CP is most often of a hypoechoic, imprecisely delineated mass in the head of the pancreas. CEUS is useful to differentiate among the two entities since pancreatic adenocarcinomas are hypoenhancing following contrast (due to massive desmoplastic reaction as well as poor vascularity), while the inflammatory pseudotumor in CP will be enhancing in the arterial phase [19][20][21].…”
