Markov modeling for the neurosurgeon: a review of the literature and an introduction to cost-effectiveness research

Abstract: OBJECTIVEMarkov modeling is a clinical research technique that allows competing medical strategies to be mathematically assessed in order to identify the optimal allocation of health care resources. The authors present a review of the recently published neurosurgical literature that employs Markov modeling and provide a conceptual framework with which to evaluate, critique, and apply the findings generated from health economics research.<… Show more

“…As the incorporation of AI and robotics continue, this new technology will fundamentally have an impact on healthcare costs to the individual and society. Approximately $3.3 trillion is spent by the United States on healthcare [57,58], and the addition of robotic systems may increase the economic burden initially. Nevertheless, the improved clinical outcomes associated with robotics may lead to an increase in overall health and quality of life, as well as a reduction in long-term costs.…”

“…[37][38][39] Markov models represent a useful approach to model cost-effectiveness. 40 Key aspects of cost analyses include describing the type of evaluation performed, how benefits and costs are assessed, the type of cost assessed, the source of the cost data, the perspective that the cost analysis is provided from, and the timeframe. [37][38][39] Economic Considerations in Neurosurgical Public Health In addition, understanding the influence of key economic considerations in the policy-making process is essential.…”

“…Adding Monte Carlo or Markov modeling to decision analysis provides a means of quantifying the degree of certainty to these estimates. The details of these techniques are beyond the scope of this review but are welldescribed elsewhere [74][75][76] and commercial software can facilitate its implementation (e.g., TreeAgePro; TreeAge Software LLC). Varying these estimates in sensitivity analyses are an important means of not only demonstrating the potential bias introduced by uncertainty in these estimates, but also which aspects have the most influence on the ultimate outcome.…”
