The use of medicinal plants is old as the existence of mankind. According to World Health Organization (WHO) data, about 80% of world population are using products based on medicinal herbs. Phytotherapy is based on the use of herbal drugs and medicinal products for the purpose of prevention and treatment. Rational phytotherapy is a modern concept of herbal medicines using, which are made of standardized herbal extracts. The quality of each final product is guaranteed by the use of raw materials of a standard quality, defined process of production, and validated equipment. Quality control of herbal drugs and herbal isolates (tinctures, extracts, and essential oils) is done according to the requirements of Pharmacopoeia and other relevant regulations. The scope of phytopreparation quality control depends on its pharmaceutical form. The formulation of a new phytopreparation is a process that has strictly defined phases: from analysis of literature and market, through defining recipes, validation of the production process, quality control of a final product to the preparation of technological and registration documents. The aim of this chapter is to present the process of herbal preparations production from selecting plant raw materials to herbal remedies (on the examples of making tea, tea mixture, drops, gels, and capsules).

The parameters of herbal drugs that are important for technological processes, especially for extraction and distillation processes were examined: bulk density, tapped density, compressibility index, Hausner ratio, and solvent absorption power. The methods described in the pharmacopoeia were used for the examination. Based on the values of bulk density and tapped density, the volume of the device is planned, ie. the size of the batch for extraction or distillation. These parameters were shown to have lower values for flower, leaf and herb, and higher values for fruit and root. Also, the compressibility index depends on the degree of fragmentation and the Hausner ratio. The results showed that these parameters have lower values for fruit and root. Solvent absorption power is the volume of solvent absorbed by a certain amount of herbal drug. Two solvents with differences in concentration were used: ethanol (aqueous solution: 96 vol.%, 70 vol.% and 50 vol.%) and distilled water. At the end of the technological process, a part of the solvent/ extract can be extracted from the spent plant material by pressing or vacuum filtration. The other part is difficult to get rid of the spent plant material and is irreversibly thrown away. In all tested herbal drugs, it is noticeable that the solvent absorption power increases with the increasing polarity of the solvent. Also, the less polar solvent EtOH 96% is more selective in terms of plant part. As the polarity of the solvent increases, it does not matter which part of the plant is extracted.

Summary Phytopreparations, in addition to the pharmacological activity and positive effects on health, can lead to side effects, toxic effects, allergic reactions, as well as the interaction of conventional and herbal medicines. Despite this, there is a generally positive attitude that herbal medicines and herbal dietary supplements are safe and harmless to health and are most often used on its own initiative, without consultations with a pharmacist or a doctor. The aim of this study was to develop and validate a questionnaire for measuring the general knowledge about phytopreparations. The study was designed as an observational, prospective cross-sectional study, intended for the valiation of the original epidemiological questionnaire to evaluate the knowledge about phytopreparations. The sample consisted of 218 respondents, who were visiting private pharmacies at the territory of Bijeljina, in February and March 2016. The final version of the questionnaire for the evaluation of knowledge about herbal preparations had good internal consistency (α= 0.849) and homogeneity when randomly splitting the questionnaire into two parts (α=0.731; 0.788). Exploratory factor analysis singled out two factors. Based on the results of the study, it can be concluded that the questionnaire was a valid and reliable instrument for the evaluation of general knowledge about safety of use of phytopreparations. This is very important because the evaluation of knowledge could lead to undertaking measures for improving it, which would reduce the potential adverse reactions and interactions of herbal preparations with conventional drugs, and the application would become safe and optimal.
