| 343 extremely simple, fast and low-cost technique, the "cyto-cell block" approach to thyroid nodule assessment may be extremely helpful in routine practice both in epithelial and in lymphoproliferative diseases.The experience of Zhang and co-authors on a large series of nodal and extra-nodal lymphoproliferative disorders studied with cell-block, demonstrated that the combination of morphology and cell block immunocytochemistry offers significant diagnostic accuracy to assess specific lymphoma subtypes, such as diffuse large B-cell lymphoma, plasmacytoma and MCL.In the present case, the immunoreactivity with cyclin D1 was per se already diagnostic of MCL. Moreover, cell block immunophenotyping provides higher diagnostic accuracy for Hodgkin's lymphoma and some T-cell lymphomas, which cannot be be diagnosed by flow cytometry. In contrast, difficulties concerning the diagnosis of marginal cell lymphoma and follicular lymphoma on cell-block specimens are related to the inability to evaluate lymphoepithelial lesions and follicular structures. 6 In summary, a cell-block specimen obtained from FNA has several advantages: it is a simple, fast and low-cost technique, it allows the morphological evaluation of the lesion, it is suitable for immunocytochemistry, it can be used for cytogenetic and molecular analysis, and, as with every paraffin-embedded tissue, it can be easily archived and re-used.
