Objective The aim of the study was the evaluation of the effects of a single manual medicine treatment (SMMT) for infants with postural and motor asymmetries in upper cervical asymmetry disorder respectively kinematic imbalance due to suboccipital strain (KISS). Methods Design: Multicentre double-blind randomised study Subjects/Setting: 202 infants at the age of 14–24 weeks with postural and movement findings were examined in four study centres using the standardized 4-item Symmetry-Score (points: 4=symmetric to 17=asymmetric). The inclusion criterion was a score of at least 10 points. The intervention group (IG) received a SMMT, whereas the control group (CG) did not receive any manual therapy. In addition, the infants of both groups were trained with a home exercise programme by their parents. Statistical Methods The primary target parameter was the result of the Symmetry-Score, measured before the intervention and 4–6 weeks afterwards. Results 171 children were randomised (IG=83/CG=88). All infants enrolled were measured to the second time-point (Intention-to-treat-analysis). The average improvement of IG compared to CG in Symmetry-Score was 2.3 points (p<.001). Following the score definition 80% of IG and 49% of CG fell below the treatment threshold of 10 points. No side effects were observed. Conclusions The SMMT significantly improves postural and motor asymmetries in infants with KISS.

Wir empfehlen ausdrücklich keine Manipulationen im Rahmen einer osteopathischen Therapie, denn auch wenn neuere Studien einen Zugewinn für den zusätzlichen Einsatz von Chiropraktik/Manualtherapie gegenüber alleiniger Physiotherapie beschreiben, gilt es, schwere Nebenwirkungen bis hin zur Asphyxie zu berücksichtigen [ 1 , 2 ]. Bei Versagen der konservativen Therapie führen wir unabhängig des Patientenalters direkt eine triterminale Tenotomie des SCM mit Absetzen der mastoidalen, sternalen und klavikulären Sehne durch, um das mögliche Rezidivrisiko zu minimieren.…”

Zusammenfassung Operationsziel Operative Korrektur des Torticollis muscularis congenitus (TMC) mit triterminaler Tenotomie des M. sternocleidomastoideus (SCM) sowie postoperativer Behandlung in der Spiegel-Orthese. Indikationen Muskulär bedingter Torticollis im Rahmen einer Kontraktur des SCM, nach Versagen der konservativen Therapie ab dem 2. Lebensjahr. Kontraindikationen Torticollis im Rahmen einer ossären Anomalie oder aufgrund anderer muskulärer Kontrakturen Operationstechnik Durchtrennung des M. sternocleidomastoideus (SCM) am Ursprung und Ansatz (klavikulär und sternal) mit Resektion der Sehne von ca. 1 cm im Bereich des Ansatzes. Weiterbehandlung Postoperative Anlage der Spiegel-Orthese. Tragen der Orthese 24 h/Tag für die ersten 6 Wochen postoperativ, danach Tragen der Orthese 12 h/Tag für weitere 6 Wochen. Ergebnisse Operiert wurden n = 13 Patienten mit TMC. Das Follow-up betrug im Durchschnitt 25,7 Monate, bei 1 Patienten kam es nach 3 Jahren zu einem Rezidiv. Weder intra- noch postoperative Komplikationen wurden beobachtet.

Factors Influencing the Duration of Rehabilitation in Infants with Torticollis—A Pilot Study

Background and Objectives: Torticollis is a common pediatric condition, with an incidence of 0.3–2.0%. Studies show that an adequate, tailored, and early treatment helps 90% to 95% of children recover before the first year of life and 97% of patients recover if treatment starts before the first six months. To identify the relationships between variables considered essential in the recovery process of infants with torticollis, we included factors such as the type of torticollis, age at onset of treatment, gender, birth weight, mode of delivery, fetal position in the uterus, the presence of craniofacial deformities, regions affected by postural asymmetries, and duration of the rehabilitation program. The hypothesis of the study is that early initiation of therapy can contribute to achieving favorable outcomes in the recovery process. Material and Methods: This retrospective cohort pilot study was conducted within a rehabilitation facility, spanning a duration of 1 year. The study involved a population of 41 children aged between 0 and 6 months. The rehabilitation program consisted of the application of Vojta therapy. Each session lasted 20 min, with a frequency of three times per week. Results: A total of 41% of those who started therapy in the first 3 months of life were fully recovered after 4–6 weeks of therapy. Of infants who started therapy at 5 and 6 months of age, 15% showed no improvement in measurements from 14 to 16 weeks of age, at which point the use of a cranial orthosis was recommended, and 23% experienced a plateau in measurements from 10 to 14 weeks, requiring the use of a cervical collar in conjunction with therapy. Conclusions: The findings from the study suggest that there may be a correlation between early initiation of therapy and favorable outcomes in the recovery process. The primary factors influencing the duration of recovery were identified as the presence of body asymmetries and the age at which therapy was initiated.
