Managing polypharmacy: teamwork in general practice

Abstract: Regular medication review has an important role in preventing patient harm from inappropriate polypharmacy. This article discusses how effective teamwork among healthcare professionals, including GPs, pharmacists and nurses working in general practice, can optimise patient outcomes.
