Revisão integrativa de literatura que tem por objetivo identificar as estratégias desenvolvidas internacionalmente para a atuação dos profissionais da atenção pré-hospitalar (APH) diante de pacientes com doença avançada em final de vida e suas famílias. As bases de dados consultadas, entre Setembro e Novembro de 2020, foram Scopus, Embase e Web of Science. O material empírico foi composto por 35 artigos, dos quais foram extraídas informações do método e dos resultados em formulário do Google, organizados em planilha no programa Excel e analisados por aproximação temática. Constatou-se que famílias e cuidadores de instituições de longa permanência apresentam medos e anseios diante da dor, da dispneia e da parada cardiorrespiratória, resultando no chamado dos serviços da APH. Os profissionais da APH, ao se depararem com a situação, possuem receios em suspender intervenções e limitações quanto à formação para os cuidados em final de vida. Dessa forma, realizam procedimentos e tendem a encaminhar os pacientes para o hospital, local em que esses acabam morrendo. Sugere-se, em relação aos cuidados com a família, o suporte diante da finitude, por meio da articulação entre os serviços que integram a rede de urgência e emergência, os quais podem favorecer o acompanhamento por serviços da atenção domiciliar ou da própria atenção primária em saúde, visando a permanência no domicílio até a morte. Ademais, demonstra-se relevante o investimento em tecnologias de informação para o compartilhamento de dados clínicos que facilitem a tomada de decisão, bem como a aproximação com serviços especializados em cuidados paliativos.

Aim This systematic review explored how non-medical factors influence the prehospital resuscitation providers’ decisions whether or not to resuscitate adult patients with cardiac arrest. Methods We conducted a mixed-methods systematic review with a narrative synthesis and searched for original quantitative, qualitative, and mixed-methods studies on non-medical factors influencing resuscitation of out-of-hospital cardiac arrest. Mixed-method reviews combine qualitative, quantitative, and mixed-method studies to answer complex multidisciplinary questions. Our inclusion criteria were peer-reviewed empirical-based studies concerning decision-making in prehospital resuscitation of adults > 18 years combined with non-medical factors. We excluded commentaries, case reports, editorials, and systematic reviews. After screening and full-text review, we undertook a sequential exploratory synthesis of the included studies, where qualitative data were synthesised first followed by a synthesis of the quantitative findings. Results We screened 15,693 studies, reviewed 163 full-text studies, and included 27 papers (12 qualitative, two mixed-method, and 13 quantitative papers). We identified five main themes and 13 subthemes related to decision-making in prehospital resuscitation. Especially the patient’s characteristics and the ethical aspects were included in decisions concerning resuscitation. The wishes and emotions of bystanders further influenced the decision-making. The prehospital resuscitation providers’ characteristics, experiences, emotions, values, and team interactions affected decision-making, as did external factors such as the emergency medical service system and the work environment, the legislation, and the cardiac arrest setting. Lastly, prehospital resuscitation providers’ had to navigate conflicts between jurisdiction and guidelines, and conflicting values and interests. Conclusions Our findings underline the complexity in prehospital resuscitation decision-making and highlight the need for further research on non-medical factors in out-of-hospital cardiac arrest.

Prehospital care has historically provided medical care with the aim of saving lives and preventing disability. Prehospital services that are accommodated by ambulances have complex and unpredictable care arrangements, so ambulance personnel, namely nurses, must prepare themselves to deal with all situations and conditions. This study aims to describe the competencies possessed by nurses while on duty in prehospital ambulances. This research is a literature study from several databases, and we obtained 20 articles for analysis. The results of the article analysis show that nurses who work in prehospital ambulances must have the competence to be able to save patients' lives, which is formed from knowledge, attitudes, and skills. Knowledge that must be possessed by ambulance nurses includes assessment, triage, and situation analysis. The positive attitudes of ambulance nurses include decision-making, communication, collaboration, caring, ethics, and culture. The skills possessed by ambulance nurses are BHD, intubation, examination of vital signs, interpretation of supporting examinations, and drug management. Ambulance nurses can improve their emergency care competencies through continuous education and training.
