Management of uraemic pericarditis.

Abstract: 564BRITISH MEDICAL JOURNAL 6 DECEMBER 1975 biotics. Polk and Lopez-Mayer25 gave cephaloridine alone in the same way, and their practice was followed by Evans and Pollock.2' Campbell28 relied on large doses of penicillin alone, and Stokes et all 9 reported encouraging preliminary results from two doses of a combination of lincomycin with either gentamicin or tobramycin. This form of prophylaxis is logical, innocuous, since side effects are unlikely from so few doses, and evidently effective. And it has wide… Show more

“…In contrast, most researchers noted suboptimal results with [12] intensification of hemodialysis prescription for dialysis pericarditis [7][8][9][10][11][12][13] . Table 7 summarizes the rates of response to intensive hemodialysis in chronic dialysis patients with dialysis pericarditis; rates of resolution range from 0 to 56% [7][8][9][10][11][12][13] .…”

“…Table 7 summarizes the rates of response to intensive hemodialysis in chronic dialysis patients with dialysis pericarditis; rates of resolution range from 0 to 56% [7][8][9][10][11][12][13] . The resolution rate, in chronological order, was 47% in 1975 [12] , 32% in 1983 [9] , 39% in 1989 [8] , 56% in 1989 [13] , 13% in 1990 [7] , 48% in 1990 [10] , 0% in 2006 [11] and 85.1% of diabetic and 82.9% of non-diabetic patients as in our study in 2008. In the largest study published concerning this subject in 1989 (n = 123), 56% of patients exhibited resolution of dialysis pericarditis following intensive hemodialysis [13] .…”

“…Table 2 summarizes rates of response to intensive dialysis and frequency of a surgical approach as found in several extensive series. These studies found suboptimal results with intensification of the maintenance dialysis prescription, with rates of resolution ranging from 12.5 to 56.1% (4,7,13,(20)(21)(22)(23). Rutsky and Rostand (4) published the most comprehensive series of ESRD patients with pericariditis in 1989.…”

“…While it may be possible to qualitatively measure the presence of the isozyme CK-MB, the percentage of this isozyme has always been the same or less than that which would have been expected to be released from skeletal muscle. 3 One major concern is the determination of what is a "substantial pericardial effusion" (moderate to large on echocardiography is in the eyes of the beholder). A significant percentage of asymptomatic patients undergoing dialysis have been shown to have small pericardial effusions and in most cases should be left alone.…”
