a b s t r a c tReconstruction of the irreparably damaged temporomandibular joint (TMJ) is dependent on the cause of damage and the patient's age. In childhood the current preference is for autogenous reconstruction which can potentially "grow" with the child. This is either with In the adult the choice is currently in favour of alloplastic reconstruction as this gives a more stable long term result and facilitates early mobilisation. Initial cost is clearly an issue, but when weighted against the reduced length of stay and reduced morbidity, often the costs are equivalent in the short term and come to benefit alloplasts in the medium term. Their long term outcomes beyond 15 years are however not clear.

The temporomandibular joint (TMJ) may be affected in patients with rheumatoid arthritis and osteoarthritis, especially in elderly and senile adult patients. The objective of this study was to evaluate the signs and / or symptoms of TMJ in elderly subjects retired from a medical institution in Merida, Yucatan, Mexico, previously diagnosed with arthritis, the onset of physical therapy and one month after applying it. July-August 2015. A longitudinal, comparative and analytical study was conducted in 25 subjects ages 60 and older, both sex: men and women. The subjects who agreed to participated signed voluntary consent letter. Also, a medical research card was made, which consisted of: 1) demographic and medical data; 2) a directed interview based on Rodriguez-Osores and modified by Mendiburu-Zavala in 2015 that included 33 dichotomous questions related to signs and / or symptoms of TMJ disorders; 3) the clinical examination was carried out carefully; 4) Finally, a manual prepared by Mendiburu-Zavala exercise based on Rocabado-Seaton and Lawson´s manual. The exercises for relaxation of the jaw and face, posture and managing TMJ noises were taught and explained individually, offering to teach them the times they needed to exercise at home. The questionnaire and clinical examination was applied a month later. The McNemar test was used. It was considered significant if P <0.05. A 28 % of the studied subject were men and 72 % women, with an average age of 65.4 ± 4.8, with a 44 % with osteoarthritis and a 56 % with rheumatoid arthritis. The effect of physiotherapy per month being applied, had a significantly favorable responses of " NO" for variables : blocked, jammed or dislocated jaw (P = 0.0156); Crackles ( P = 0.0195 ) and Snap ( P = 0.0001 ). The application of physiotherapy in this age group with arthritis was favorable.

Background: Rheumatoid arthritis (RA) is an autoimmune disorder that usually affects joints and making them warm, painful, and swollen. The chief purpose of this study was to evaluate movements of mandible in geriatric patients suffering from RA with or without temporomandibular joint dysfunction. Materials and methods:A total of 45 people were included in this study with age of 60 years and above. Partially or completely edentulous patients were divided into experimental and control group. Experimental group consists of 20 people suffering from arthritis and control group consists of 25 people without arthritis. Movements were recorded with or without prosthesis while mastication in both experimental and control group.Results: Statistical evaluation of two studied groups showed decrease opening angle (p < 0.05) during mastication; however, insertion of new prosthesis showd a significant increase in values in both groups, with an increase in opening and closing angles. Conclusion:Positive correlation was found between arthritis and movements of the mandible in older people suffering from RA.Clinical significance: Patients suffering from RA are having restricted mandibular movements thus imposing an overall negative impact however; presence of prosthesis has been shown to enforce a positive effect on mandibular movement.
