Management of the Patient with a Malignant Pleural Effusion

Abstract: A malignant pleural effusion (MPE) establishes an incurable stage of a malignancy. Median survival after detection of an MPE is 4 to 6 months in general populations of patients with cancer. Management of MPE centers on palliation of symptoms because no available treatments prolong survival. Mismanagement of MPE, however, can shorten survival and add to patients' burden of disease. Appropriate management requires a multidisciplinary approach with competency in existing treatment modalities to allow individualiz… Show more

“…In this case, pleural biopsy or thoracoscopic surgery will normally be indicated for more accurate evaluation of malignant pleural involvement and metastasis. However, these invasive approaches require general anesthesia and may induce significant patient morbidity and increased health care costs and, thus, sometimes are not a good option for patients with advanced-stage disease (23).…”

“…The production of MPE is a pathological process, which results from the failure of pleural defense mechanisms and abnormal mesothelial function, and it is defined by the presence of tumor cells in the pleural effusion [18]. Pulmonary carcinoma is one of the main causes of MPE [19,20].…”

“…Plevral sıvı pH'ı ne kadar düşük ise plöredezisin başarılı olma şansı o kadar azdır. pH 7.30'un üzerinde ise başarı olasılığı %90'dan fazla iken pH 7.15'in altında olduğunda başarı olasılığı %50'nin altındadır (30) (24,31) . Plöredezis için birçok kemoterapötik ve diğer kimyasal ajanlar değişen başarı oranları ile uygulanmaktadır.…”
