Pain is a serious problem globally due to its high prevalence and is regarded as the fifth vital sign. The standard methods comprising of NSAID and opioid group, interventional methods and implantable methods; although often very effective, have known adverse effects on gastrointestinal, renal and cardiovascular systems and surgical risks. Ayurvedic pain management comprises of pharmacological and non-pharmacological approaches. In this review, we have discussed pain management using herbal and herbomineral formulations. We began with comparison of Ayurvedic concept of pain with that of Biomedicine to arrive at concurrence and then systematically searched the databases PubMed and Scopus using search terms 'pain management, animal models of pain, Ayurveda and herbal medicine'. Out of 160 initial papers, 16 studies involving a total of 1591 participants befitting inclusion criteria were analyzed for the type of pain, intervention, outcome measures and results and others were rejected. PRISMA is followed to report the findings. Studies on Ayurvedic medicines were conducted in three different countries. Eight of them were RCT's with five multi-centered studies. Three single herbs, six polyherbal formulation, one herbomineral formulation and various external therapies were identified. The studies used standard scales like VAS or WOMAC index for analyzing the intensity of pain and showed statistically significant results to Ayurvedic medications in various pain conditions without any adverse effects. This review gives a comprehensive understanding of the concept of pain and its management from both Biomedicine and Ayurveda. It also highlights the unexplored potential of many herbomineral formulations to treat pain.

Background: Thermal Microcautery (TMC) is a form of peripheral nerve field stimulation and is a technique used in Traditional Indian Medicine (Agnikarma) to manage chronic pain. The aim was to asses TMC in painful knee osteoarthritis (PKO). Methods: A non-randomized controlled trial was employed. All PKO patients on a waiting list for knee replacement were offered PNFS-TMC. Patients in the intervention group received 4 sessions (2 weeks apart) of PNFS-TMC by a pinpoint tip of a metal rod. The rod was heated over a flame for 5 minutes before being used to induce a single 1 mm second-degree burn over points of tenderness. The control group was selected from the same waiting list. The primary outcome was assessed by Visual Analogue Scale (VAS) scores. The secondary outcomes were changes in levels of physical day to day activity, sleep and analgesics. Results: 16 PNFS-TMC intervention group patients and 15 control group patients were subject to analysis. Baseline VAS score was higher in PNFS-TMC group [9 ± 1.23 (95% CI 8.38-9.61) versus 5.93 ± 2.11 (95% CI 4.81-7.06) in the control, P < 0.0001]. VAS scores after 8 weeks (i.e. 4 sessions) were lower in the PNFS-TMC group [4.64 ± 2.08 (95% CI 3.57-5.72) versus 6.73 ± 2.01 (95% CI 5.61-7.85) respectively with P = 0.0058]. In the PNFS-TMC group, VAS score decreased from 9 ± 1.23 (95% CI 8.38-9.61) at baseline to 4.64 ± 2.08 (95% CI 3.57-5.72) after 4 sessions (P < 0.0001). In control group, VAS score showed a non-significant increase in VAS score from 5.93 ± 2.

Diagnosis and Management Pattern of Lateral Epicondylitis in a Tertiary Care Center

Introduction: Lateral Epicondylitis has been found to be the second most frequently diagnosed musculoskeletal disorder. A wide range of symptomatic treatments are available such as use of anti-inflammatory analgesic drugs, steroids, physiotherapy. This study aims to know about the diagnosis, prescription pattern and current practice on management of tennis elbow in Nepal. Methods: This is a hospital based observational study carried out at Bir Hospital, Kathmandu, Nepal. Patients diagnosed with tennis elbow were purposively selected through prospective sampling technique from Orthopedic Department. Questionnaire and patient medication files were used as tools for data collection. Results: A total of 97 patients were found to be suffering from tennis elbow affecting mostly 41-50 years of age group and seen mostly in female (62%). Further, it was found that housewives (31%) were mostly affected. Diagnosis of tennis elbow was done commonly by clinical evaluation (61%) and X-ray (39%). Both Pharmacological and Non-Pharmacological approaches were in practice. Pharmacological treatment include NSAIDS (59% Aceclofenac, 19% Naproxen, 18% Indomethacin, 16% Diclofenac, 6% Piroxicam) and Steroids (23% methylprednisolone acetate and 21% oral prednisolone). Non-Pharmacological treatment was done by lifestyle modification (100%), 78% application of heat, 63% use of tennis elbow band, 29% exercise and 28% physiotherapy. Surgical intervention (3%) was also done when the conservative management failed. Conclusions: There is professional risk of tennis elbow for housewives, farmers and shopkeepers in context of Nepal. Only one treatment approach is not effective in management of tennis elbow for long term effect. Keywords: lateral epicondylitis; NSAIDs; physiotherapy; steroid; tennis elbow.
