Management of Shoulder Injuries Using Dry Needling in Elite Volleyball Players

Abstract: These case reports describe the short-term benefits of dry needling in shoulder injuries in four international female volleyball athletes during a month-long intense competitive phase, using both replicable subjective and objective measures. Dry needling of scapulohumeral muscles was carried out. Range of movement, strength and pain were assessed before and after treatment, with a functional assessment of pain immediately after playing and overhead activity, using the short form McGill Pain Questionnaire. “…While many of these studies directly use the term ‘dry needling’,5,18,21,26,33,56,64,67,104,110 in their methodology, other studies use alternative terminology such as ‘deep dry needling’,67 ‘superficial dry needling’,55,128 ‘TrP acupuncture’,20,80 ‘paraspinal needling’,64 ‘intramuscular and nerve root needling’,65 ‘needle electrical intramuscular stimulation’,129 ‘needle release’,113 ‘acupuncture needling’,63,113 ‘needling therapy’,17 ‘acupuncture’36–38,40,41,47,49–55,61,69,72,8188,90,92,94100,105108,112,115117,119123,130135 and ‘electroacupuncture’ 37,46,93,126,127,136139. “…Fibroblasts not only produce proteins that make up the extracellular matrix, but also transform into myofibroblasts to repair injury via production of collagen and α smooth muscle actin protein. 36 Findings from research studies on the mechanisms of action underlying the effects of needling not only explain why needling per se is effective for musculoskeletal pain treatment, [29][30][31][32][33][34][35] but also account for the growing use of DN in the musculoskeletal field including sports medicine. Needling may thus improve muscle performance, although large, high quality research studies are needed to determine the optimal parameters of needling, including location and direction of needle insertion, duration of needle retention, the requirement for a local twitch response or de qi sensation, the frequency of treatments, and its potential role as a preventive measure.…”

“…2 Dry needling has been demonstrated to be an effective intervention for patients with chronic shoulder pain and ROM deficits. 18 A case series by Osborne and Gatt 33 showed improved shoulder ROM and function in elite-level volleyball players after dry needling of the scapulohumeral muscles. In a case series by Ingber, 19 3 patients with shoulder pain were treated with dry needling of the shoulder-region muscles.…”
