Management of sensitized pediatric patients prior to renal transplantation

Abstract: The four sensitized patients reported here who received desensitization therapy had successful renal transplants with a low risk of immediate post-transplant rejection. Overall, long-term allograft functions and complications from immunosuppression were encouraging.

“…In the United States, approximately 3% of pediatric first‐time renal transplant recipients are already sensitized with PRA >80%, due to prior exposure to blood transfusions or other organ transplants. This increases their risk of hyperacute and acute rejection following subsequent solid organ transplantation . Single small pediatric kidneys may be particularly susceptible to this phenomenon, as well as medication noncompliance, which is known to occur commonly in adolescence …”

“…In kidney paired donation (KPD) programs, this allows access to better HLA‐ or blood group–matched living donors, and in the Eurotransplant Acceptable Mismatch Program, highly sensitized patients are given priority for organs from deceased donors with HLA alleles that are at low risk of producing a positive crossmatch . While both desensitization and ABO‐mismatched transplantations have been successfully performed for children , KPD offers access to LDKT without the associated higher immunosuppression required to overcome these immune barriers.…”

“…The manuscript by Priojsakul et al [4] is therefore a welcome addition to the literature. Unfortunately, there are a number of concerns with this paper.…”

“…Priojsakul and colleagues [4] describe four cases, of which two were living donors: one had a positive cross-match with a low level of DSA and one a negative baseline cross-match and no significant DSA. It is unclear whether a subsequent positive B cell flow cytometric cross-match could have been due to autoreactivity or administration of intravenous immunoglobulin (IVIg).…”
