Management of recurrent intra corneal epithelial cyst with ethanol irrigation and vacuum-assisted cyst wall excision

Abstract: The clinical and pathological features and management of a patient with recurrent intracorneal epithelial cyst are reported. A child presented with a large intracorneal cyst and underwent drainage with 96 % ethanol irrigation. Histopathology confirmed the epithelial nature of the cyst. The cyst recurred, however, and subsequently a repeat ethanol irrigation with removal of the cyst wall was done. The cyst wall was vacuumed to ensure complete removal of epithelial cells. There was no recurrence, with good visua… Show more

“…Further knowledge about its extent can be gained from ultrasound biomicroscopy. 4,10 Yet, the definite proof is obtained by histopathology of the cyst specimen lined by epithelial cells. Recently, confocal microscopy has emerged as a diagnostic tool, which reveals in vivo corneal structure, layer by layer, in healthy and diseased individuals.…”

“…Surgical approach can provide both the diagnosis and treatment. Emptying the cyst of its content by marsupialization 3 or aspiration 4 and the removal of its walls by excision, 2,5 keratoplasty, 6,7 and by peeling yields epithelial cells to be identified by histopathology and enables the stromal repair by apposition of the corneal lamellae. Instead, the cyst may collapse after a discharge of its liquid content through a spontaneously created opening in the anterior corneal stroma, and the in vivo histopathological diagnosis can be made by using in vivo confocal microscopy (IVCM).…”

“…Common causes of traumatic cysts include penetrating or perforating wounds and surgical trauma. 1 In cases without an overt history of trauma, undetected previous injury should be considered. 5 Cysts have been reported from birth to aged 76 years, and the involved eye is usually quiet and noninflamed.…”
