Management of periprosthetic distal femur fractures using distal femoral arthroplasty and fixation - Comparative study of outcomes and costs

Abstract: Our study compares the outcome and cost of distal femoral arthroplasty to that of Fixation (Plating/Retrograde Nailing) in the management of distal femur peri-prosthetic fractures. Methods: We reviewed our database for patients admitted with peri-prosthetic distal femoral fractures between 2005 and 2013 (n ¼ 61). The patients were stratified into 2 groups based on management method. The Distal Femoral Arthroplasty group (Group A) had 21 patients and the Fixation group (Group B) had 40 patients. Outcome & cost … Show more

“…With regard to complications, a direct comparison study observed no difference between the two groups of patients with post-operative complications; however, one in five patients in the fixation cohort developed a non-union [ 3 ], which is consistent with the literature regarding non-union rates [ 7 ]. Another study reported that, at one year, there were no complications in the DFR cohort compared to one failure of fixation, six malunions, and two infections in the fixation cohort [ 12 ]. A study examining a group of patients treated with DFR found implant-related complications in 11% or in two cases of their sample.…”

Tandon et al 6 suggest that distal femoral arthroplasty may be a suitable alternative to osteosynthesis for periprosthetic fractures around Total Knee Arthroplasty among patients in the eighth decade of life.…”
