Management of Mirizzi syndrome: A new surgical approach

Abstract: Although, out of 26 choledochoplasties, we encountered only one (3.84%) stump stone in a maximum follow-up period of 59 months, further long-term follow-up studies are required to prove the efficacy of the procedure.

“…Stone recurrence in gallbladder remnant (stump stone) is a potential drawback of our technique, but we did not find this complication in this series during an average follow-up period of 12.1 months. In a study conducted by Shah et al, only one stump stone (1/26, 3.84 %) occurred in a maximum follow-up period of 59 months [29]. Compared with biliary stricture, a possibly lethal complication that may occur after a bilioenteric bypass [25], stone recurrence is a relatively minor complication and can be effectively treated with ERCP and EST.…”

“…Moreover, only case reports and small series can be found in the literature, and there is no difference between the open and laparoscopic era in the rate of its detection [1,2,4,5,7,8,12,14,22,25,[27][28][29][30]. In these reports, the incidence of MS ranged from 0.4% to 2.53% [1,2,4,5,7,12,14,25,[27][28][29]. Several classification systems have been developed to distinguish between different types of MS. Involvement of the hepatic duct via the destruction of its wall or simple external compression is the most important criterion.…”

“…The remnant of the gallbladder required is a cuff of 5 to 10 mm depending on the size of the fistula [19]. There have been several series of successful laparoscopic treatments for types I and II presentations [4,12,16,17,22].…”
