Management of leg ulcers with hydrocolloid occlusive dressing

Abstract: Twenty-two patients with 31 leg ulcers of various causes received local wound management consisting of the application of an adhesive hydrocolloid (HC) occlusive dressing (DuoDerm). Group 1 comprised 15 patients (11 outpatients) with 19 ulcers. Approximately two thirds (12/19) of the ulcers healed when treated solely with the HC dressing; mean treatment time was 57 days. Group 2 comprised seven hospitalized patients with at least two leg ulcers of similar size and distribution. The 20 ulcers in the seven patie… Show more

“…An ideal wound dressing is one, which induces host cells to regenerate, prevent infection, act like an osmotic regulator, 1 providing optimal environment for healing to take place quickly. 2 Although occlusive dressings possess clinical benefit in treatments of wounds, 3,4 they may provide a favorable environment for the microflora of wound to further proliferate and grow. 5 Microbes, entering the wound, delay healing by initiating infection, leading to wound fluid exudation, and have a number of deleterious effects on the healing process.…”

“…62 It also occludes the wound, prevents exposure of denuded nerve terminals to air, and, thus, mitigates the tingling characteristic of shallow wounds. 63 • Commercially available HD materials, which are translucent and allow observation of the wound after application, include DuoACTIVE â ET. Visiderm â (a translucent and waterproof polyurethane film using HD in the layer adhering to the skin) and Remois â Pad (a transparent polyurethane film having a 3-layer structure with the layer adhering to the skin being HD compounded with moisture-retaining ceramide 2, supportive layer above it being a polyurethane film, and the outermost layer above it being very slippery nylon knit with a low friction coefficient) may also be used, though they are not medical materials.…”

“…One of these dressings is the hydrocolloid dressing. [3][4][5][6][7][8] These opaque, absorbent dressings consist of hydrocolloid granules embedded in an adhesive matrix covered by a polyurethane foam. In the Western world, these dressings are now widely used in the management of (leg) ulcers.…”
