Management of Lacrimal Gland Carcinoma

Abstract: Given the rarity of lacrimal gland carcinoma, multi-institutional studies and consistent reporting of size and histologic type of tumors in the literature may be prudent. Particularly, multimodality globe-sparing treatment strategies should be studied further.

“…The most common benign tumor of the lacrimal gland is the pleomorphic adenoma, while the adenoid cystic carcinoma (ACC) is the most common malignant tumor[7,9,11]. Therefore, most studies on epithelial tumors of the lacrimal gland have been predominantly on ACC[12,13]. The subtypes of lacrimal gland carcinoma, other than ACC, are very rare and sebaceous carcinoma is one of the rarest tumor among them[12].…”

“…The primary treatment for benign lacrimal gland tumors is surgical resection, whereas malignant tumors are generally removed surgically followed by radiotherapy and/or chemotherapy [8]. Hadron beam therapy has also been considered a nonsurgical treatment option for patients not suitable for surgical treatment [9]. However, there is some controversy surrounding therapeutic options, particularly for malignant tumors.…”

“…As compared to other lacrimal gland carcinomas, patients with ACC of the lacrimal gland are typically diagnosed at younger ages, report earlier and more frequent periocular pain, and experience local recurrence or distant metastasis even after long quiescent periods [3]. The difficulties in localized control and the high tendency for regional recurrences in ACC are attributed to perineural infiltration and bone invasion [4]. Distant metastasis is seen in up to 50% of patients with ACC, with the lungs, bone, and liver reported as the most commonly affected sites [1,5,6,7].…”
