Management of infantile esotropia

Abstract: Although many theories exist, the true cause of infantile esotropia remains unknown. The literature suggests that treatment before age 2 and perhaps even earlier improves the potential for binocular vision. After significant review of literature, it is clear that surgery is the treatment of choice for infantile esotropia but no method has a clear advantage. Efforts to define the best surgical procedure and timing of surgery through prospective, randomized multicenter trials are warranted.

“…The focal point of this treatment is surgical intervention. 4 Nevertheless, due to the lack of randomized trials comparing different surgical strategies for IE, the most effective one and the optimal timing have not yet been univocally defined. 3 According to scientific literature, surgical alignment of eyes in IE has a variable success rate reported to be from 45% to 80%; 5 nevertheless, there are still few data regarding risk factors associated with the need of reoperation.…”

“…This period can extend for many years, potentially into adulthood, but with an exponential decrease in plasticity associated with increasing age. The severity of the loss is greatest during the early development, which is clearly demonstrated in children with infantile esotropia . In this type of strabismus the onset is before six months of age, meaning the development of stereopsis is interrupted at a very early stage following a period of time when the eyes were aligned.…”

“…The potential for the recovery of stereopsis is pivotal to the argument relating to the timing of correction in infantile esotropia. Reports do agree that the results are compelling showing treatment before the age of two years (and preferably younger) results in better levels of stereoacuity . Given that the rates of recovery of stereopsis are lower beyond a limited three month time frame between onset and correction, it is argued that it is beneficial to delay treatment until a later age, where there is less risk for a consecutive exotropia to develop.…”

“…Erken cerrahi ile daha iyi stereopsis seviyesi elde edilebileceği görüşü ile birlikte erken cerrahiyi savunan oftalmologların sayısı artmaktadır . Erken cerrahi ile daha iyi binokülerite bildirilen çalışmalarda önerilen cerrahi zamanı farklı çalışmalarda ilk 6 ay (10,13) , ilk bir yaş (18) ve ilk 12-18 ay (15)(16)(17) olarak değişiklik göstermektedir.…”
