Management of hypertension in patients with coronary artery disease

Abstract: Hypertension is present in three of four patients with coronary artery disease (CAD) but remains largely uncontrolled in most patients. Treating hypertension in these patients is complicated by the concern of precipitating coronary ischemia when diastolic blood pressure (DBP) is reduced excessively. However, an emerging body of evidence from recent clinical trials in high-risk hypertensive patients with or without CAD demonstrated the benefit of intensive drug therapy, even when DBP fell much lower than 80 mm … Show more

“…The prevalence of non-communicable diseases (NCDs) has increased significantly worldwide (Vongpatanasin, 2008), and they are the most important cause of mortality and morbidity in Iran (Azizi et al, 2002;Sarraf-Zadegan et al, 1999). Based on the Tehran Lipid and Glucose Study (TLGS) findings, the prevalence of general obesity between the years 2006 and 2009, classified by BMI ≥30, was 21% and 38.6% in men and women respectively, demonstrating an alarming status and calling for urgent action to implement effective lifestyle modifications (Hosseinpanah, Barzin, Eskandary, Mirmiran, & Azizi, 2009).…”

“…During the past century, CVD has become one of the major contributors to the global burden of disease [2] and CVD is now responsible for 30% of all deaths worldwide [1]. Epidemiological data from the US suggests that hypertension may be present in up to 75% patients with IHD [3]. Despite a large number of recent trials examining antihypertensive agents in patients with IHD or at risk for IHD, the optimal choice of individual antihypertensive agents in the prevention and management of IHD remains controversial.…”
