Management of grade III giant cell tumors of bones

Abstract: Our results suggest that surgical curettage with various adjuvant modalities might be considered as the first choice for treatment of high-grade GCTs in weight-bearing areas, especially in young patients, to avoid unnecessary endoprosthesis.

“…Extension of curettage has been achieved using a highspeed burr combined with adjuvant to preserve joint function, and this method has been associated with a low rate of recurrence (4%-14%) [5,6,12,13,23,25,27,36,44,48,52,53,56]. Therefore, our objective was to identify cases of GCTs of the distal radius to determine whether curettage is associated with lower rates of recurrences and fewer major complications compared with en bloc excision, and whether recurrence rates differed with the use of PMMA versus bone graft in patients treated with curettage.…”

“…About 2 to 10% of patients developed distant metastasis, usually in the lung. 5 The local recurrence rate is high after conservative treatment. The rate decreases to <10% after curettage and adjuvant treatments.…”

“…Methotrexate-loaded bone cement (rather than simple cement) minimises the amount of tumour-induced osteolysis and is useful for treating pathologic fractures. 5 The histological grade of tumour has little value in predicting treatment outcome. 8,11 The rate of local recurrence correlates more with the treatment method.…”
