Management of Complications Related to Removal of the Buccal Fat Pad

Abstract: Removal of the buccal fat pad (BFP) is an important topic of discussion in the literature. Several studies have reported improvements in facial esthetics as a result of this technique. The BFP is close to vital structures, such as the facial nerve, parotid duct, and vessels. Injuries related to these structures may occur during the surgical procedure. This manuscript aimed to report and discuss 2 clinical cases of the complications after removal of the BFP. Besides the case presentation, a comprehensive review… Show more

“…In the case described here, traumas and aesthetic goals were present as procedure indications [8]. The intraoral approach is safer and could be performed with minimal complications (8.45%-18%) [5]. It is crucial to comprehensively discuss risks and bene ts and to obtain informed consent [3,9].…”

“…Hemorrhage is the most feared complication due to the risk of hemorrhagic shock from lesions to the facial artery or other vessels derived from the external carotid artery [3,7,25,28]. Other complications include infection [20], lesions to the salivary parotid duct or stula [14,28], loss of nerve function [14], and incomplete removal [5].…”

“…However, the procedure is not without risk. Complications include damage to the buccal branch of the facial nerve, to the parotid ducts, uncontrolled bleeding, pathological scarring, among others [8,20,23]. In some cases, surgeons may overestimate the amount of buccal fat, due to a lack of parameters and indications for the procedure [10].…”

“…"The anatomical knowledge of this region is essential to avoid iatrogenesis in surgical procedures, which can result in temporary or permanent sequelae" [15]. "Among the complications of greater complexity in buccal fat pad removal there are: trismus, hemorrhages, facial infections, lesion of the duct of the parotid gland and facial paralysis, however common complications that are related to any surgical procedure can occur, such as edema and hematoma, despite being considered a technically simple procedure" [2].…”
