Management of Chronic Heart Failure Guided by Individual N-Terminal Pro–B-Type Natriuretic Peptide Targets

Abstract: Serial NT-proBNP measurement and targeting to an individual NT-proBNP value did result in advanced detection of HF-related events and importantly influenced HF-therapy, but failed to provide significant clinical improvement in terms of mortality and morbidity. (Effect of NT-proBNP Guided Treatment of Chronic Heart Failure [PRIMA]; NCT00149422).

“…is prototypical. The PRIMA study by Eurlings et al 213 enrolled 345 patients and targeted discharge NT-proBNP after HF hospitalization or the observed lowest NT-proBNP in the 2 weeks after hospitalization. This more "individualized" approach was intended to minimize overtitration of therapies in patients with persistently elevated NT-proBNP.…”

“…In some studies, a prespecified treatment escalation algorithm was followed21, 22 while in other studies the attending clinician was instructed to escalate treatment according to their individual clinical judgment 23, 24, 25. Our study used an empirical prespecified treatment escalation strategy that, included increasing existing doses of furosemide followed by introduction of thiazide diuretics and spironolactone, similar to the algorithms described in the aforementioned human studies.…”

“…Elevated MMP‐9 levels may help identify patients at risk before an increase occurs in ventricular pressure overload, which reflects ongoing ventricular remodeling. The value of BNP levels for guiding therapy in addition to clinical symptom‐based treatment seems to be limited,9, 10, 11, 21 despite the undisputed diagnostic and prognostic importance of these values 6, 7, 8. The benefits of predicting HF events may be offset by non‐HF events.…”

“…Of these, elevated plasma concentrations of brain natriuretic peptide (BNP) show notable associations with development of left ventricular dysfunction and HF 5. Although elevated BNP concentrations have diagnostic and prognostic importance,6, 7, 8 whether intense therapy for HF guided by BNP values is superior to therapy guided by clinical symptoms is unclear 9, 10, 11. BNP play important roles in maintaining cardiorenal homeostasis under physiological and pathological conditions.…”
