Background: Transtibial amputation and subsequent prosthetic fitting can cause prolonged hospital length of stay (LOS). Recently, a two-staged admission pathway for new transtibial amputees was introduced in a local hospital to shorten hospital LOS. Methods: A retrospective cohort study was performed to review the 5-year outcomes of major lower extremity amputation in a local hospital and to compare the difference in LOS and functional outcomes between traditional (group 1) and two-staged (group 2) admission pathways for new transtibial amputees. Results: The fitting rate was 55% in transtibial amputees, 14% in transfemoral amputees, and only 3.7% in dementia patients. For new transtibial amputees, the mean postoperative LOS was significantly reduced by 32.8 days in group 2. Besides, more patients in group 2 were able to walk without aid. Conclusion: LOS for new transtibial amputees can be greatly reduced by the two-staged admission pathway to minimize hospital operational cost.

Introducción: La amputación de miembro inferior es una de las principales causas de discapa¬cidad permanente, la que se asocia en más del 80% a complicaciones de la diabetes mellitus. En Chile existe poca información en relación a las características clínicas y demográficas de los pacientes amputados en los hospitales públicos. Materiales y métodos: Estudio observacional, descriptivo y retrospectivo. La población fue pacientes amputados de miembro inferior atendidos por fisiatría del Servicio de Salud Metropo¬litano Occidente, mientras que la muestra se tomó de pacientes con amputación de miembro inferior nivel transtibial y transfemoral atendidos por fisiatría en el Hospital San Juan de Dios entre diciembre de 2019 y marzo de 2020. Resultados: La base de datos contenía información de 75 pacientes, siendo el 88% de género masculino. La media de la edad fue de 59,6 años. El mayor número de amputaciones fue de tipo unilateral (82,6%), predominantemente de lateralidad derecha y nivel transtibial. La principal causa de amputación fue complicaciones de la diabetes mellitus en un 85,3%. Solo el 17,3% adquirió prótesis, la mayoría de forma particular. Solo un 14,7% se reinsertó laboralmente. Discusión: La limitada adquisición de dispositivos protésicos es un obstáculo para la rehabili¬tación de pacientes con amputación de miembro inferior, aspecto que incide directamente en la independencia en la marcha y en la reinserción laboral de los pacientes. Sería interesante realizar un estudio de costo-efectividad entre el financiamiento de prótesis versus los meses de licencia médica que debe subsidiar el estado.
