Malignant Transformation of Angiomyolipoma

Abstract: Angiomyolipomas are well recognized but uncommon tumors that can involve the kidney. To our knowledge a well documented case of malignant change of angiomyolipoma has never been reported. We report a case of renal angiomyolipoma and leiomyosarcoma with a clearly identified transition of benign to malignant elements seen in the resected tissue.

“…This variant often displays aggressive behavior. 12,13 Epithelioid angiomyolipomas with metastatic lesions or sarcomatous transformation have been reported 14,15 and are referred to as malignant epithelioid angiomyolipoma. In our case, multiple nodules of epithelioid angiomyolipoma occurred in the liver and hepatoduodenal ligament after resection 11 years earlier.…”

“…It was difficult for us to make a definitive diagnosis of epithelioid anigiomyolipoma on frozen section and imprint preparation in the intraoperative consultation, as the tumor cells showed severe atypia and mitotic figures. So far, only one report describing the cytologic findings of epithelioid angiomyolipoma has been documented, 12 and epithelioid angiomyolipoma occurring in the peritoneal cavity has not been reported. Here we report the cytologic findings of an imprint preparation.…”

“…However, due to the preoperatively diagnosed tumor Schips/Ratschek/Gallé/Zigeuner/Trummer/ Petritsch/Hubmer thrombus and the size of the tumor with involvement of almost the whole kidney, radical nephrectomy was performed. As cases of angiomyolipoma with malignant transformation have been reported in the literature [6,7], we also had to suspect malignancy in our patient; however, this was excluded pathologically.…”

“…Große Raumforderungen können allein durch ihre Größenzunahme mit Verdrängung von Gefäßen sowie ihrer Nähe zu den Nieren und Harnwegen weitere Komplikationen hervorrufen. Es wurde auch ein seltener Fall von maligner Entartung eines Angiomyolipoms beschrieben [10]. Differentialdiagnostisch muß hier insbesondere immer ein Liposarkom mitbedacht werden [15].…”
