Malignant transformation in non-irradiated juvenile laryngeal papillomatosis

Abstract: A spontaneous cancer transformation of non-irradiated juvenile papillomas has been observed in a very small number of cases. We report on six more patients with juvenile laryngeal papillomatosis developing into a squamous-cell carcinoma. Three patients had childhood papillomatosis and three patients adult onsets of their papillomas. The average duration between onset of papillomatosis and cancer diagnosis was 33 years, while the average age at time of diagnosis of a laryngeal cancer was 50 years. All patients … Show more

“…Recurrent respiratory papillomatosis can, however, be protracted by the need for recurrent surgical interventions to avoid airway obstruction and can extend below the vocal cords, sometimes as far as the main stem bronchi; cases of parenchymal lung involvement have been described (recently reviewed by Harada et al [1]). In addition, a number of cases of malignant transformation either at the laryngeal [2,3], bronchial [4], or pulmonary level [5] have been reported.…”

“…There were no patients in our series with histologically verified malignant degeneration of papilloma which developed after a certain follow-up period. Therefore, we can speculate that due to the reasons mentioned above the rate of possible malignancy of AP in our series was higher (12.3%) as referred to the literature [14,18,21].…”

“…Laryngeal stenosis is rarer than in JP. Data from the specialist literature indicate that approximately 1.7-7% of AP cases are liable to become malignant [14,18,21].…”

“…Die Eingriffe erfolgen heute in aller Regel unter Einsatz eines CO 2 -Lasers, mit dem die Papillome teils abgetragen, teils vaporisiert werden können [2,141]. Eine histologische Aufarbeitung reprä-sentativer Teile der Papillome muss in jedem Fall angestrebt werden, weil auch die rezidivierende Kehlkopf-Papillomatose gelegentlich maligne entarten kann [142].…”
