Malignant Proliferating Trichilemmal Tumors—Report of Three Cases

Abstract: We are reporting three patients with recent history of rapid enlargement of long standing nodular scalp lesions. Histologic sections revealed areas of relatively benign proliferation of pilar sheath epithelium and foci of trichilemmal keratinization characteristic of a proliferating trichilemmal cyst. There were also extensive areas of severe cellular dysplasia and surrounding dermal invasion indicating malignant transformation.

“…Malignant transformation from proliferating trichilemmal tumours can also occur in rare instances. Mehregan and Lee 11 indicated that rapid growth or exophytic enlargement of the long‐standing nodule and histologically extensive areas of severe dysplasia and surrounding dermal invasion are signs of malignant transformation. Although our patient could not tell us the exact duration of each tumour, she did admit that tumours showing typical trichilemmal cyst histopathology were of short duration, changed in morphology and did not disappear.…”

“…15,23 Malignant pilar tumors range from in situ foci within an otherwise classical pilar tumor to frankly invasive neoplasms with a destructive growth pattern. 24 Therefore, it was not considered in the differential diagnosis of the present case, which was histologically a typical example of a trichilemmal (pilar) cyst.…”

“…In some cases of PTT, atypical areas were found to be admixed with well-differentiated lobules [18]. Moreover, cases with little or no cytological atypia displayed aggressive behavior and vice versa [19].…”
