Malignant Melanoma 2003

Abstract: The incidence and mortality of melanoma has seemed to level off for certain groups after a steady increase during the last 50 years. This trend is suspected to be secondary to education efforts aimed at prevention and better detection and removal of thin, biologically benign melanomas. Since no effective systemic therapies exist for metastatic melanoma, early detection and removal of thin melanomas offer the best chance of cure. For thicker melanomas, sentinel lymph node biopsy has improved the accuracy of sta… Show more

“…This observation is consistent with an increased resistance of melanotic melanomas to radiotherapy or chemotherapy and the immunoevasive properties of melanogenesis [21, 23, 3638]. In agreement with above findings, we have found that induction of melanin pigmentation is associated with decreased VDR expression and decreased responsiveness to active vitamin D compounds [18, 21].…”

“…In agreement with above findings, we have found that induction of melanin pigmentation is associated with decreased VDR expression and decreased responsiveness to active vitamin D compounds [18, 21]. Therefore, we propose that melanogenesis [36] or synthesis of the pigment intermediates [23] affects VDR signaling in melanomas with a final attenuation of their responsiveness to vitamin D.…”

“…Though necrosis and age are considered as prognostic factors for malignant melanoma,[30] there was not a significant relationship between necrosis and age and depth of the tumor in this study. It may be due to the small sample size.…”

“…Biopsy is essential to establish a definitive diagnosis. The histological characteristics of melanoma include pagetoid intraepidermal spread, absence of maturation, increased cellularity and/or dermal mitotic figures [23] . No single histologic characteristic is diagnostic of melanoma.…”
