“…The same thing was described long ago after reduction or withdrawal of azathioprine, prednisone, and antilymphocyte globulin 16,17 and documented in detail by Iwatsuki et al 18 and Geis et al 5 Of the 5 patients reported by Geis 5 and Iwatsuki 18 and their associates whose lymphomas regressed, 1 later acquired a different neoplasm and the other 4 remained tumour free. 20 As with kidney and liver recipients, the only survivors among the 5 heart or heart-lung recipients in the present series were 2 who had withdrawal of immunosuppression. “…Hence, doses of ALG, prednisone, and other cytotoxic agents may be augmented needlessly. Be sides further complicating the immunohematological problem at hand, high doses of antilymphocyte globulin would increase the risk of adverse consequences of immunosup pression, including infection [1,7,11] and possibly neoplasia [10,20], (2) Transfusion in the face of incompatible crossmatches due to MALG antibody alone would appear to be safe, if performed cau tiously. Negative results of screening proce dures for red cell antibodies, performed be fore institution of therapy with MALG, strengthen the validity of this premise in any individual case.…”
