Malignant Fibrous Histiocytoma of the Spermatic Cord: The Role of Retroperitoneal Lymphadenectomy in Management

Abstract: Malignant fibrous histiocytoma of the spermatic cord is rare, with only 6 previously reported cases. We describe 2 new cases that were treated with retroperitoneal lymphadenectomy. To our knowledge, this is the first report in the literature of retroperitoneal lymphadenectomy for the treatment of this specific lesion. The literature is reviewed in regard to the sarcomatous nature of this tumor and the use of retroperitoneal lymphadenectomy performed for spermatic cord sarcomas. We conclude that this procedure … Show more

“…RPLND is required for those rare cases of tumors with retroperitoneal nodal metastasis identified on imaging studies [20,25], intermediate or high-grade malignant fibrous histiocytoma and fibrosarcoma [23,26] and according to the pathological specimen (e.g. if capsular invasion into the cord tissues is evident).…”

“…Retroperitoneal lymph node dissection is advocated in cases with rhabdomyosarcoma, intermediate or high-grade MFH, and fibrosarcoma, on the basis of pattern of relapse and low probability of salvage of retroperitoneal recurrence (3,4,21) and according to the pathological specimen, if capsular invasion into the cord tissues is evident (4), or lymphadenopathy is evident in CT studies (17). In our series this procedure was performed in two patients with rhabdomyosarcoma, and in two patients with other histologies.…”

“…In adults, while radical orchiectomy is considered the procedure of choice for spermatic cord STS, retroperitoneal lymph node dissection, as well as adjuvant radiation therapy and chemotherapy remain controversial treatments (1)(2)(3)(4). The paucity of literature data makes treatment decisions more complicated.…”
