The goal of all medical treatments is a better quality of life for patients. Post-stroke rehabilitation is a long process with uncertain result. The aim of this study was to explore the factors which affect the quality of life of patients recovering from a cerebrovascular disease. This is a prospective study evaluating the quality of life of one hundred patients one month and six months after a stroke, and patients also answered questions retrospectively, of how they felt before the stroke. As assessment tools we used a questionnaire on general and clinical data and Medical Outcomes Study Short Form (SF-36) questionnaire. Physical functioning and Physical role domains of SF-36 show significant differences in both measured periods (p<0.001). Emotional role, Social functioning, Mental health, Vitality and General health domains show a statistically significant change during first six months, while Bodily pain domain did not change (p>0.05). Physical summary score has changed significantly during 6 months (p <0.001). Mental summary score showed no significant difference in both periods (p <0.687; p <0.958). The brain localization is important factor (p<0.0002). Gender, age, education, employment status and previous strokes did not have a statistically significant influence (p> 0.05). Post-stroke physical impairment is not always accompanied by emotional impairment. Emotional functioning impairments generally return to the premorbid level during the period of six months, while physical impairments continue to occur. Further research is needed for better understanding of these relationships.

BackgroundAfter discharge from hospital following a stroke, prescriptions of community-based rehabilitation are often downgraded to “maintenance” rehabilitation or discontinued. This classic therapeutic behavior stems from persistent confusion between lesion-induced plasticity, which lasts for the first 6 months essentially, and behavior-induced plasticity, of indefinite duration, through which intense rehabilitation might remain effective. This prospective, randomized, multicenter, single-blind study in subjects with chronic stroke-induced hemiparesis evaluates changes in active function with a Guided Self-rehabilitation Contract vs conventional therapy alone, pursued for a year.MethodsOne hundred and twenty four adult subjects with chronic hemiparesis (> 1 year since first stroke) will be included in six tertiary rehabilitation centers. For each patient, two treatments will be compared over a 1-year period, preceded and followed by an observational 6-month phase of conventional rehabilitation. In the experimental group, the therapist will implement the diary-based and antagonist-targeting Guided Self-rehabilitation Contract method using two monthly home visits. The method involves: i) prescribing a daily antagonist-targeting self-rehabilitation program, ii) teaching the techniques involved in the program, iii) motivating and guiding the patient over time, by requesting a diary of the work achieved to be brought back by the patient at each visit. In the control group, participants will benefit from conventional therapy only, as per their physician’s prescription.The two co-primary outcome measures are the maximal ambulation speed barefoot over 10 m for the lower limb, and the Modified Frenchay Scale for the upper limb. Secondary outcome measures include total cost of care from the medical insurance point of view, physiological cost index in the 2-min walking test, quality of life (SF 36) and measures of the psychological impact of the two treatment modalities. Participants will be evaluated every 6 months (D1/M6/M12/M18/M24) by a blinded investigator, the experimental period being between M6 and M18. Each patient will be allowed to receive any medications deemed necessary to their attending physician, including botulinum toxin injections.DiscussionThis study will increase the level of knowledge on the effects of Guided Self-rehabilitation Contracts in patients with chronic stroke-induced hemiparesis.Trial registrationClinicalTrials.gov: NCT02202954, July 29, 2014.

Methodology: an exploratory study characterized as a research of literature review conducted through electronic review in scientific databases in the Virtual Library for Health Highwire Search, PubMed, Sielo, EBsco Host, held in November and December 2010. Found 12 full papers, were 5 in Highwire Search, 6 in EBsco Host and 1 in PubMed. Results: in the articles analyzed were not found protocols that, in a systematic way, guide the practice of evaluation, diagnosis, intervention and reassessment of care to patients with incontinence after stroke. Conclusion: Although the studies addressing important aspects of the patient's urinary incontinence after stroke, evaluation and identification of factors that enhance the situation, can make an important contribution to the selection of strategies and the definition of assistance for the recovery of continence. The evaluation of incontinence and these factors must be done systematically and using validated instruments for this purpose. Descriptors: rehabilitation; protocols; stroke; nursing; urinary incontinence RESUMO Objetivo: rever a literatura sobre protocolos clínicos focalizando as diferenças e semelhanças relacionadas com a incontinência urinária pós AVC. Metodologia: estudo exploratório caracterizado como pesquisa de revisão de literatura realizada por meio de revisão eletrônica nas bases de dados científicas da Biblioteca Virtual de Saúde Highwire Search, PubMed, Sielo e EBsco Host, realizada em Novembro e Dezembro de 2010. Foram encontrados 12 artigos completos sendo 5 na Highwire Search 6 na EBsco Host e 1 no PubMed. Resultados: nos artigos analisados não foram encontrados protocolos que, de forma sistematizada orientem as práticas de avaliação, diagnóstico, intervenção e reavaliação dos cuidados ao doente com incontinência após AVC. Conclusão: apesar de os estudos tratarem de aspectos importantes da incontinência urinária do doente pós AVC, a avaliação e identificação de factores que potenciam a situação, podem dar um contributo importante na selecção das estratégias e na definição das intervenções com vista à recuperação da continência. A avaliação da incontinência e desses factores deve ser efectuada de forma sistemática e com recurso a instrumentos validados para esse fim.
