Purposes:To assess the efficacy of laparoscopic nephrectomy for a single-system ectopic ureter draining a dysplastic kidney in children. Patients and Methods: Between February 1999 and September 2005, 16 girls with a mean age of 6.2 years (range: 2-15 years) presented with urinary incontinence accompanied by regular voiding since birth (15 patients) and vaginitis (one patient). Ultrasonography, intravenous urography and a 99m Tc-DMSA renal scan showed the presence of only a single kidney in all cases. Computed tomography (CT) showed a dysplastic kidney definitely in nine patients, structures suspicious of dysplastic kidney in three cases, and no dysplastic kidney in four cases. Magnetic resonance imaging was carried out in the four cases with non-visualized dysplastic kidneys by CT, and showed a suspicious lesion in only one case, and no lesion in the other three patients. All patients underwent transperitoneal laparoscopic nephrectomy for a dysplastic kidney. Results: Laparoscopy identifies all dysplastic kidneys easily, even in those cases in which dystrophic kidney could not be identified by preoperative imaging. Dysplastic kidneys and ectopic ureters were removed successfully in all 16 patients. Mean operative time was 109 min (range: 40-155 min) with little intraoperative bleeding. Mean postoperative hospital stay was 2.6 days (range: 2-4 days). No intraoperative complication was encountered , except in one single case, in which a small bowel injury occurred during open Hasson's procedure. All patients became dry soon after the operation. Conclusion: Laparoscopic nephrectomy for an ectopic ureter draining into a dysplastic kidney is a safe and effective method , and can be carried out successfully, despite a failure by preoperative imaging studies to localize the dysplastic kidney.

Continuous dribbling of urine is the major symptom in girls with an ectopic vaginal ureter. However, the diagnosis is somewhat difficult in girls with such an entity before toilet-training has been achieved. The current concept in the management of these entities is to remove the renal moieties with the vaginal ectopic ureter drain. We report on a case of an obstructed vaginal ectopic ureter associated with a duplicated collecting system which was managed successfully by renal-sparing surgery.

Children are not small adults. In the urinary tract, particularly, there are many conditions that are more or less specifi c to children which need early and reliable diagnosis in order to provide proper treatment to prevent renal damage and severe long-term sequelae such as hypertension or renal insuffi ciency. This is particularly important for all congenital urinary tract malformations and also for many other congenital renal conditions -these are often detected prenatally and, after birth, need prompt and reliable diagnostic workup at minimal invasiveness. Additionally, even in conditions that are the same or similar to adults, the specifi c needs and conditions in childhood require different imaging approaches and algorithms, partially because of each of the following reasons: the child's higher sensitivity to radiation, different physiology, different size and imaging appearance, as well as different potential of the various imaging techniques. These differences are outlined, focusing on some important diseases and queries such as congenital hydronephrosis and obstructive uropathy, impact of vesicoureteral refl ux on the kidney, imaging in infants with urinary tract infection, childhood urolithiasis and nephrocalcinosis, pediatric renal masses, as well as other miscellaneous renal conditions that may manifest in the child and indicate imaging. Additionally, specifi c needs for adapting and applying the various imaging techniques in childhood are discussed.
