Magnetic Resonance-Guided High-Intensity Focused Ultrasound (MR-HIFU): Overview of Emerging Applications (Part 2)

Abstract: Background High-intensity focused ultrasound (HIFU) allows noninvasive heating of deep-seated tissues. Guidance under magnetic resonance imaging (MR-HIFU) offers spatial targeting based on anatomical MR images as well as MR-based near-real-time temperature maps. Temperature feedback allows delivery of a well-defined thermal dose enabling new applications such as the ablation of malignant tissue. Methods Peer-reviewed publications on MR-HIFU were studied and are summarized in this review. Literature… Show more

“…The temperature feedback allows the direct delivery of a defined dose of thermal ablation to the malignant tissue. Nowadays, new transducers are developed to provide the treatment in several malignancies, including prostate or breast cancers [72]. Other indications of MRgFUS/MR-HIFU use might include the palliation of pain due to tumour occurrence or metastasis by producing tissue denervation, tumour mass reduction and neuromodulation [73].…”

“…Recently, a model predictive control algorithm in combination with MR temperature mapping was introduced for a voxel-based heating strategy that allows compensation for local heat losses e.g., due to perfusion, with a high spatial resolution [39]. MR thermometry finds now routine use in all MR-HIFU applications [388,389].…”

“…The most relevant clinical applications of this approach concern tissue ablation, mostly in the context of tumor treatment. Brief overviews of this group of applications, directed to clinicians, have been published in the past decade, hinting at brain, breast, prostate, and liver tumors as the primary targets (Colen et al, 2017;Cornud et al, 2018;Siedek et al, 2019aSiedek et al, , 2019b. It has also been pointed out that thermal ablation of nonneoplastic central nervous system tissue is a valid option, e.g., for patients suffering from chronic neuropathic pain (thalamotomy).…”
