MAFLD identifies patients with significant hepatic fibrosis better than NAFLD

Abstract: Background & Aims Diagnostic criteria for metabolic associated fatty liver disease (MAFLD) have been proposed, but not validated. We aimed to compare the diagnostic accuracy of the MAFLD definition vs the existing NAFLD criteria to identify patients with significant fibrosis and to characterize the impact of mild alcohol intake. Methods We enrolled 765 Japanese patients with fatty liver (median age 54 years). MAFLD and NAFLD were diagnosed in 79.6% and 70.7% of patients respectively. Significant fibrosis was d… Show more

“…In another study, Yamamura et al conducted a careful and detailed analysis and demonstrated in a cohort of 765 Japanese patients with fatty liver that the MAFLD criteria identifies patients with metabolic dysregulation, chronic kidney disease and significant hepatic fibrosis substantially better than NAFLD, independent of the amount of alcohol intake 39 . In this work, authors utilised a decision tree classifier approach which is a powerful data‐mining method to identify the priorities of factors and calculates their importance 40 …”

“…8 This novel concept and criteria enable clinicians to identify more patients at risk of adverse outcomes in clinical practice. 9,10 Since this concept was introduced recently, the utility of MAFLD in clinical practice requires further investigation. As the diagnosis of MAFLD can be achieved by fulfilling either one, two or all of the three metabolic conditions, there is no clarity whether MAFLD patients with different numbers or types of metabolic conditions have different risks of liver fibrosis.…”

“…The significant difference between NAFLD and MAFLD is that the diagnosis of MAFLD does not need to exclude other etiology of liver disease, such as excessive alcohol intake or viral infection[ 4 ], while the presence of metabolic dysfunction is necessary for MAFLD[ 5 ]. These criteria help to identify more cases at high risk[ 6 , 7 ].…”
