Objective. To develop a set of core outcome measures for use in randomized controlled trials (RCTs) and longitudinal observational studies in juvenile idiopathic arthritis (JIA)-associated uveitis. Methods. The literature relating to outcome measures used in studies of uveitis in childhood and adolescence was reviewed. A set of core outcomes and domains was established using the Delphi process. This was reviewed by a representative multinational interdisciplinary working group. Nominal group technique consensus was reached on face and content validity of the range and content of the domains. The outcomes and the appropriate instruments for uveitis trials were adapted to the age ranges of patients with JIA-associated uveitis. Results. Consensus was reached that data should be reported at defined time points in longitudinal studies with patients stratified by prognostic markers. Visual acuity testing should be age appropriate. The severity of uveitis (measured as anterior chamber cell grade) and duration of active inflammation should be documented. Visually significant structural complications should be recorded and quantified with standard measures. The responses to treatment and corticosteroidsparing effects of treatment should be documented. Patient-reported disease activity and age-specific uveitis-related quality of life should be reported using appropriate questionnaires. Conclusion. The proposed outcome measures in JIA-associated uveitis should aid in the standardization and comparison of future RCTs of the treatment regimens for this disease. The proposed outcome measures will be verified in a prospective validation study.

ÖzetAmaç: Akut ön üveitli (AÖÜ) gözlerde aktif inflamasyon döneminde ve iyileşme sonrası optik koherens tomografi (OKT) ile makula ve peripapiller retina sinir lifi (RSL) kalınlığını değerlendirmek ve hastaların sağlıklı diğer gözleriyle karşılaştırmak. Ge reç ve Yön tem: Hastanemiz uvea biriminde tek taraflı AÖÜ teşhisi konan 32 ardışık hasta prospektif olarak çalışmaya alındı. Hastaların 'Stratus OCT fast makuler thichness map' programı ve 'fast RNFL thickness' programı ile OKT'leri çekildi. Fovea merkezli 1 mm, 3 mm ve 6 mm'lik daire içindeki ortalama makuler kalınlık ile total makuler volume (TMV) ve peripapiller 3,4 mm'lik halkada ortalama RSL kalınlıkları AÖÜ'lü ve sağlıklı gözler arasında karşılaştırıldı. Ayrıca atak anında alınan ölçümler, iyileşme sonrası aynı gözden alınan ölçümlerle de karşılaştırıldı. Sonuçlar: Olguların 14'ü kadın, 18'i erkek olup yaş ortalaması 37,0 (20-65) yıl idi. Hastaların aktif inflamasyonlu gözlerinde sağlıklı gözlere göre OKT ile elde edilen 1 mm, 3 mm ve 6 mm'lik halkalardaki makula, TMV ve peripapiller RSL kalınlıklarında istatistiksel olarak anlamlı artış tespit edildi (p<0,05). İnflamasyon varlığında 3 ve 6 mm'lik halkadaki maküler kalınlık ve TMV, inflamasyonun kaybolduğu vizitteki değerlere göre istatistiksel anlamlı şekilde daha yüksekken (p<0,05), peripapiller RSLT açısından anlamlı fark bulunmadı (p>0,05). İyileşme sonrası alınan ölçümlerle hastaların sağlıklı gözleri karşılaştırıldığında 3 mm'lik ve total makula kalınlıklarının anlamlı şekilde halen yüksek olduğu saptandı. Tar t›fl ma: Sonuç olarak AÖÜ hastalarında arka segmentte klinik olarak inflamasyon bulguları saptanmasa dahi OKT ile makula kalınlığında ve peripapiller RSLT'de artış olmakta, tedavi ile makula kalınlığı azalmakta ancak klinik bulgular kaybolsa bile makuladaki kalınlaşma bir süre daha devam edebilmektedir. Ma te ri al and Met hod: Twenty-two consecutive patients diagnosed with unilateral AAU in the Uvea Department of our hospital were included in this study. OCT measurements were taken with 'Stratus OCT fast macular thickness map' program and 'fast RNFL thickness' program. Mean thicknesses of fovea centered 1 mm, 3 mm, 6 mm circles, total macular volume (TMV) and mean RNFL thickness in 3.4 mm peripapillary circle were compared between eyes with AAU and healthy eyes. Also, the measurements during the acute attack were compared with measurements of the same eyes after recovery. Re sults: There were 14 women and 18 men with a mean age of 37 (range, 20-65) years.
