Maculopathy in uveitis of juvenile idiopathic arthritis: an optical coherence tomography study

Abstract: Among children with JIA uveitis, macular involvement is frequent and characterised by perifoveolar thickening and serous retinal detachment. OCT is a non-invasive instrument. It can easily identify this maculopathy, which could impair visual function, and require therapeutic intensification.

“…Maculopathy has been documented by OCT in 84 % of eyes with JIA-associated uveitis [57], a much higher figure compared to previous reports mostly based on ophthalmoscopic findings (3.9-26 %) [61][62][63][64]. Macular changes determined by OCT included perifoveolar thickening in 74 %, macular edema in 48 %, foveal detachment in 18 %, and atrophic changes in 10 % of eyes with JIA-associated uveitis [57].…”

“…However, eyes with serous retinal detachment and macular edema responded favorably to uveitis treatment at 3 and 6 months, achieved greater rates of improvement and resolution of macular edema than eyes without serous retina detachment, and recovered to a similar level of visual acuity. An unusual finding described in eyes with active iridocyclitis using Stratus OCT is the occurrence of a ring-like pattern of retinal thickening outside the foveal center in the inner ring and combined inner and outer rings [55][56][57]. The macular thickening has been shown to have a significant and consistent correlation with active iridocyclitis [55].…”

“…It has probably been underestimated prior to the widespread use of fundus lenses and optical coherence tomography (OCT). In 1 study, macular edema was found in 84% of cases of JIA-associated uveitis (26). There was consensus among the participants that the presence or absence of macular edema should be documented as an outcome measure.…”
