Macular Hole Formation in Rhegmatogenous Retinal Detachment after Scleral Buckling

Abstract: PurposeTo describe early macular hole (MH) development in rhegmatogenous retinal detachment (RRD) after scleral buckling (SB) based on optical coherence tomography (OCT) findings.MethodsThe medical records and spectral domain OCT images of patients in whom MH developed after RRD repair were evaluated retrospectively.ResultsA postoperative MH was detected in five eyes that underwent SB during a 6-year period. All had fovea-off RRD without MH at the time of surgery. OCT showed partial loss of the inner retina wi… Show more

“…In cystoid macular oedema and during early stages of macular hole formation, fluid‐filled cysts may be present in the foveola resulting in a detachment of the inner Müller cell layer; the detached Müller cell layer often keeps the foveal walls together (Gass ; Byon et al. ). Disruption of the inner Müller cell layer of the foveola results in the formation of full‐thickness macular holes (Gass ).…”

“…assumed previous PVD and macular-off RRD causing fragility of the foveal tissue and following phagocytosis of damaged foveal tissue result in macular hole formation. [ 12 ] It is not known whether PVR may play a role in subsequent FTMH after recurrent RRD. The PVR may have contributed to the residual tractional forces following vitrectomy for RRD repair that may have been responsible for macular hole formation.…”

“…Secondary MH formation following RRD repair surgery was first documented in 1988 [ 1 ]. This rare phenomenon carried prevalence varying from 0.32 to 0.96% [ 2 ]. Macular-off RRD and repeated surgery for RRD seemed to be important risk factors of this phenomenon [ 2 , 3 ].…”

“…Tangential traction caused by PVD [ 2 ], residual vitreous, and ERM were thought to cause anteroposterior traction of the macula, and lead to MH. However, there were cases of MH formation without evidence of posterior cortical vitreous traction after VT for RRD [ 4 ] and even after ERM removal.…”
