Macular Buckling for Eyes With Myopic Foveoschisis Secondary to Posterior Staphyloma

Abstract: Combined pars plana vitrectomy and macular buckling is an effective approach to treat myopic foveoschisis with both anatomical and visual improvement. Despite a similar complication rate with both buckling techniques, macular buckling with the Ando plombe was found to be technically easier with shorter surgical times.

“…We also agree with Mateo et al [25] that macular buckling should be considered in eyes with posterior staphyloma and great axial length. A macular plombe reshapes the posterior scleral wall and releases the traction caused by the posterior staphyloma, in combination with vitrectomy, to avoid the anteroposterior traction caused by vitreous cortex or tangential traction caused by the premacular membrane or ILM.…”

“…In 2006, Baba et al [26] used macular buckling alone to treat cases with foveoschisis and foveal detachment, achieving a visual improvement of two or more lines in 66% of the eyes. Mateo et al [27,28] showed similar results in a large series using macular buckling combined with vitrectomy ( fig. 4).…”

“…4 In 2010, Kumagai 45 In 2012, Mateo and Corcostegui et al described using vitrectomy with a macular buckle in 16 eyes with documented progression on MF and reduced visual acuity, and obtained an improvement in acuity of 87.5%. 39 MF resolution on OCT was achieved in all patients but was observed to occur earlier in those treated with the Ando plombe compared with the hard silicone implant for posterior buckling.…”
