Il presente articolo è già stato pubblicato sulla rivista "Farmeconomia e percorsi terapeutici" nel terzo numero del 2006. Per completezza lo riportiamo anche in questo supplemento dedicato al ruolo delle statine, e in particolare di atorvastatina, nella riduzione del rischio cardiovascolare.ipertensione, dislipidemia e problemi della coagulazione [5]. Inoltre, nei pazienti diabetici, aumenta il rischio di malattie coronariche e cerebrovascolari. L'infarto acuto del miocardio, ad esempio, si manifesta con una frequenza da due a tre volte maggiore nei soggetti diabetici rispetto ai non diabetici, mentre il rischio di ictus e di sviluppare malattie vascolari periferiche è ancora più elevato.L'associazione americana per il diabete ha stimato che negli USA nel 2002 globalmente sono stati investiti 132 miliardi di dollari, dove il 70% è da attribuirsi a costi diretti.Per quanto concerne l'Europa lo studio CODE (Cost of Diabetes in Europe-type 2) è nato con lo scopo di stimare in otto nazioni (Belgio, Francia, Germania, Gran Bretagna, Italia, Olanda, Spagna, Svezia) i costi della gestione di pazienti con diabete di tipo 2, dal quale è emerso chiaramente che il diabete richiede cospicue INTRODUZIONEIl peso economico del diabete è sicuramente rilevante per la società. Tale peso è attribuibile a due circostanze: la gestione del diabete in se stesso ed il trattamento delle sue complicanze [1,2]. Il diabete mellito è una malattia molto diffusa associata ad un incremento del rischio di malattie concomitanti e di morte. Si stima che in Europa siano affetti da diabete mellito più di 18,5 milioni di persone [3], mentre la prevalenza di diabete diagnosticato in Italia è del 3% circa [4]. Da un recente studio condotto in Italia risulta che l'incidenza del diabete mellito è di 2,2 per 1000 persone per anno (gli uomini 2,0 e le donne 2,4) [4].Per quanto riguarda la manifestazione delle complicanze associate alla malattia, esistono numerosi fattori di rischio che contribuiscono allo sviluppo di malattie macrovascolari in soggetti affetti da diabete mellito, tra cui iperglicemia, ABSTRACT Introduction: cardiovascular diseases (CVD) are the major component of premature mortality, generate disability and are a relevant source of cost.
