Machine learning approaches for prediction of early death among lung cancer patients with bone metastases using routine clinical characteristics: An analysis of 19,887 patients

Abstract: PurposeBone is one of the most common sites for the spread of malignant tumors. Patients with bone metastases whose prognosis was shorter than 3 months (early death) were considered as surgical contraindications. However, the information currently available in the literature limits our capacity to assess the risk likelihood of 3 month mortality. As a result, the study's objective is to create an accurate prediction model utilizing machine-learning techniques to predict 3 month mortality specifically among lung… Show more

“…For palliative pain relief, patients in the high-risk group may benefit from the best supportive care, short-term radiation, or even minimally invasive procedures like cementoplasty ( 7 ). The study’s proposed model, which dose not ask for any additional staff training, can be used clinically to forecast the survival benefit of patients with bone metastases and raise the performance of oncologists and radiologists who aren’t professionals to that of experts.…”

“…Surgery shouldn’t be performed on patients who have a low chance of survival since it could cause more harm than good. In particular, palliative therapies should be used to treat patients with a survival time of fewer than three months ( 7 9 ). It should be noted that early appropriate prognostic discussions with patients could lead to better medical education of therapeutic goals and life expectancy ( 10 ).…”

“…The calibration slope and intercept-in-large value were obtained from the calibration curve. Additionally, a scoring system was used to comprehensively evaluate the prediction performance of the models 29 , 30 , with each metric rated on a scale of 1 to 6. The scoring system ranged from 0 to 60.…”

“…All patients entered our database after their diagnosis of bone metastases owing to cancer. Early death was defined as patients who died at or within 3 months [ 15 , 16 ] after the diagnosis of bone metastases, and the death was attributed to causes related to their cancer. T and N stages were recorded based on the combination of the American Joint Committee on Cancer and Extent of Disease classification.…”

“…In addition, the clinical usefulness of the models was evaluated using decision curve analysis after calculating the net benefits in a range of threshold probability. The metrics were scored by sorting them based on their prediction performance, as per the findings of a previous study [ 16 ]. On the basis of each evaluation metric, the highest score was assigned to the best-performing model, followed by the next best model.…”
