Lymphoma After Solid Organ Transplantation: Risk, Response to Therapy, and Survival at a Transplantation Center

Abstract: EBV-naïve patients who receive a donor organ from an EBV-infected donor are in the highest-risk situation for PTLD development. Most of these lymphomas are CD20 positive. Follicular lymphoma is unusual. With treatment, survival of patients with PTLD was indistinguishable from that of the SEER population sample.

“…(Faull et al, 2005;Webber et al, 2006).More recent data suggest that the median time is later, at approximately 3 years after SOT. ( Evens et al, 2010;Ghobrial et al, 2010;Knight et al, 2009;Maecker et al, 2007) This is partly due to an increase in the diagnosis of EBV-unrelated disease. Multiple reports have shown that EBV-unrelated PTLD occurs later after SOT, most commonly after 3 to 5 years.…”

“…(Dotti et al, 2002;Leblond et al, 1998;Nelson et al, 2000;Taylor et al, 2005) Recent data shows an improved response to treatment, which could be explained both by improvements in treatment strategies of the tumors and by improved supportive care. “…Since PTLD arise in only 1.0 ± 0.3% of all transplant recipients, [22][23][24] it is likely that other conditions have to be present to allow the emergence of the disease (e.g. a particular cytokine milieu, as reviewed by Burns and Crawford 25 ).…”

“…Posttransplant lymphoproliferative disorders (PTLDs), a group of B-cell (and less frequently, T-cell) lymphomas, develop as a complication of immunosuppression in patients undergoing organ transplant [1]. The first report of PTLD was in 1969 [2] by Penn and colleagues, but since then, it has been reported after different kinds of organ transplant and has become an entity of interest and research worldwide.…”
