Lymphocytic Hypophysitis Accompanied by Aseptic Meningitis Mimics Subacute Meningoencephalitis

Abstract: A 66-year-old man was admitted to our hospital with muscle pain, fatigue and appetite loss that had lasted for a month. The patient was somnolent and had nuchal stiffness with a high fever. Cerebrospinal fluid (CSF) tests demonstrated lymphocytic pleocytosis with a decreased CSF-blood glucose ratio. The tests for tuberculous, fungal and carcinomatous meningitis and herpetic meningoencephalitis were negative. Endocrinological exams showed hypopituitarism and diabetes insipidus. A pituitary MRI showed an enlarge… Show more

“…1,12 An additional risk factor for LAH is aseptic meningitis. 13 Several risk factors for secondary forms of hypophysitis have also been identified; these are summarized in Box 2. Fukuoka sera are allowed to react with several sections of human surgical specimens, rodent pituitaries, rodent pituitary cell lines, or baboon pituitaries, and antihuman IgG is used as a secondary antibody.…”

“…Peripartum period and aseptic meningitis have been established as risk factors for primary hypophysitis [6]. The underlying pathophysiology has an autoimmunity basis in the peripartum period, since hyperestrogenism produces an increased volume and perfusion of the gland, exposing it to the immune system [2].…”

“…The underlying pathophysiology has an autoimmunity basis in the peripartum period, since hyperestrogenism produces an increased volume and perfusion of the gland, exposing it to the immune system [2]. The finding of antipituitary antibodies, albeit with low sensitivity and specificity (26 and 36% respectively) [1][2][3][4][5][6][7], supports this theory.…”
