Lymph node ultrasound during melanoma follow-up significantly improves metastasis detection compared with clinical examination alone

Abstract: Early detection of melanoma metastases is essential for effective treatment and may be crucial for the prevention of systemic metastases and patient survival. However, data demonstrating the reliability and accuracy of ultrasound examination for the detection of lymph node metastases, in addition to clinical examination, are rare. We have examined 433 melanoma patients with stage-dependent follow-up intervals of 3 to 12 months. One thousand three hundred and thirty-two paired clinical and nonblinded sonographi… Show more

“…Correct diagnosis the nature of cervical lymph nodes is extremely important from the therapeutic viewpoint. One of the major treatment choices for metastatic cervical nodes is neck dissection, which may lead to complications, whereas the treatment for tuberculosis lymphadenitis is anti-tuberculosis medication, which is far less complicated than surgery (Krüger et al, 2011). Ultrasonography (US) is noninvasive and can give useful clues in the diagnosis of cervical lymphadenopathy, however, the differential diagnosis is difficult in some situations even combined with color Doppler imaging (Khanna et al, 2011).…”

“…For patients with thin melanoma, advances are likely to come from the development of new biomarkers to identify those patients who may forego an SLNB [46][47][48]. For patients with melanoma where the chance of a positive node is low, high-resolution ultrasound may be a viable alternative to SLNB, particularly for head and neck melanoma where technical difficulties make SLN biopsy unreliable [49][50][51].…”
