“…Although the 6 th edition of the American Joint Committee on Cancer (AJCC) TNM classification is an excellent classification system and has been extensively used for gastric cancer staging worldwide (Greene, 2002), different lymph node staging systems between the Japanese Gastric Cancer Association (JGCA) classification and the AJCC classification had made it difficult to compare treatment outcomes in an international setting (Japanese, 1998;Ikeguchi et al, 2004;Aurello et al, 2007). In Aug, 19 th , 2008, the member of AJCC, UICC and JGCA held the World Cancer Congress in Buffalo, New York, which aimed to revise and unify stage system of gastric cancer.…”

“…3,4 However, limitations such as lack of information regarding the extent of lymph node dissection and difficulty in its application to surgical treatment planning have been demonstrated as disadvantages of a numeric classification system. [5][6][7] Moreover, to the best of our knowledge, there has been no consensus regarding the adequate grouping of lymph node staging. Another current issue in gastric cancer staging concerns the classification of depth of invasion, especially for the pT2 classification.…”

“…The defi nition proposed by the AJCC/UICC is based on the absolute number of positive lymph nodes, while that put forward by the JGCA is based on the location of positive lymph nodes. Advantages exist in both systems [1,3] . Recently, a growing body of evidence in the literature [4][5][6] suggests that the AJCC/UICC staging system may predict prognosis more precisely and that the rate of positive lymph nodes is an independent predictive factor of prognosis [7][8][9] .…”

“…The definition of regional lymph nodes is the principal difference and the main controversy [1,2] . The defi nition proposed by the AJCC/UICC is based on the absolute number of positive lymph nodes, while that put forward by the JGCA is based on the location of positive lymph nodes.…”
